2345874 Primary Facility 2015-02-11 Routine/Complete Inspection No Violations Noted Falcon Drilling Rig #25 was in the process of moving to the location during the inspection. We met with Rudy Holtzclaw, company man of SWN Energy. Mr. Holtzclaw stated that drilling will most likely commence tomorrow, 2/12/15. The 2H-5H wells have been spudded and will be drilled to kickoff point with Falcon Rig #25. The surface and intermediate casing of these wells will be cemented. Most likely a Precision Drilling Rig will mobilize to the site at a date to be determined to re-enter the wells to drill from kickoff point to total depth. The 6H well is in the production phase of operations. There are (2) heating units staged on the pad surface near the production fluid tank containment. Mr. Holtzclaw claimed the tanks did not contain any diesel fuel and the tanks will be placed on containment upon filling with fuel.
2349563 Primary Facility 2015-03-06 Drilling/Alteration No Violations Noted Inspected the Tonya West 3H on March 6 2015. Halliburton was on location to cement surface string from 0' to 833'. Crew got circulation, pumped 100 bbls gel followed by 181 bbls of cement at 15.8 ppg. 57 bbls of good cement weighing 15.8 ppg was brought to surface. Plug was bumped on calculated volume, floats held with 3/4 barrel back to truck after releasing pressure.
2349752 Primary Facility 2015-03-04 Drilling/Alteration No Violations Noted Inspected Southwestern's Tonya 2H at 11am on 03/04/2015. Falcon 25 just drilled and skidded to the 3H. the 2H had 826' of cemented surface casing and 1621' of cemented intermediate casing in the hole. Both strings had good cement to surface, casing wasn't disturbed during set time.

2418234 Primary Facility 2015-10-20 Routine/Complete Inspection No Violations Noted On-site, Mr. Klemish and I met with Dave Singer, SWN Completions Foreman overseeing Halliburton’s hydraulic fracturing operations. Hallibuton is in the process of fracking 4 wells, the 2H, 3H, 4H, and 5H. The inspection revealed that the “dust” in question was actually frac sand that was becoming air borne as it was off loaded to proppant storage tanks. The Halliburton team working this part of the operation stated that they were having issues with their filtration system and that they had additional components in route and that they were actively trying to fine tune the operation. The airborne frac sand appeared to be coming from multiple locations of this operation. The “dust” on-site became irritating to me near the off-loading area which caused me to move back to the completion foreman’s trailer. At that time, I stated that fugitive emissions off-site of an operation were a violation of the Department’s Air Resources regulations.
2422912 Primary Facility 2015-10-26 Routine/Complete Inspection No Violations Noted Inspection conducted on 10/26/15 of the Tonya West Pad as a follow-up inspection to an ongoing dust complaint related to the sand off loading at the site as part of the hydraulic fracturing activities. Present during the inspection was Joe Gaboff, Department OGI. The dust issues that are related to the off-loading of the frac sand were not observed during the inspection. Joe Gaboff believes that the sand capture system must not be working efficiently as evidenced by the amount of filled tote bags present near the off-loading site. It appears as if the filtration system must be very labor intensive, again as evidenced by the high volume of frac sand being collected. There are no longer any hatches left open. The Department will continue to conduct intensive investigations to ensure that the short cuts observed during the initial investigation of this site do not resume.
2422947 Primary Facility 2015-11-05 Routine/Complete Inspection No Violations Noted Inspection conducted on 11/5/15 of the Tonya West Pad as a follow-up inspection to an ongoing dust complaint related to the sand off loading at the site as part of the hydraulic fracturing activities. The hydraulic fracturing activities have been completed. The frac sand off-loading dust suppression system has been removed. The containment is still in place. A high volume of waste sand is still present on the containment. Coil tubing activities taking place during the inspection. No violations noted. Please remove the waste sand that is present in order to avoid natural causes, i.e. wind from mobilizing the sand.
2425079 Primary Facility 2015-11-16 Routine/Complete Inspection No Violations Noted During the inspection, Ryan Klemish, SWN Regulatory Analyst and Dan Jones, Acting Regulatory Analyst arrived on-site. Mr. Klemish also metered this location with a pen meter and recorded a reading of 3 milliSiemens/cm³. The Department was able to collect a small sample of this water and is having it analyzed for the metal analytes of the Department’s 946 suite. The second puddle in question had an elevated reading of 6 milliSiemens/cm³. This puddle is located right off containment in front of the well heads on the entrance side. The Department spoke to Mike Deming, Senior Regulatory Analyst by phone about the puddles in question. Mr. Deming agreed to have a consultant evaluate both locations.
2426683 Primary Facility 2015-11-19 Routine/Complete Inspection No Violations Noted Mr. Deming stated he would work on getting the pump system up and running ASAP. As always, please be prepared to provide the Department with a detailed report of your field screenings as had been conducted on the hot spot near the well heads. Mr. Deming stated that there is a possibility that dust suppression activities conducted in response to a dust complaint on the access road may be contributing to the elevated conductivity readings. He is going to attempt to contact the contractor and get a sample of the pure product that was used on the access road.
2432485 Primary Facility 2015-12-14 Administrative/File Review No Violations Noted A review of the Department collected sample results as illustrated in the attached Table 1, reveal that a spill has occurred at the well site, and it is recommended that SWN should conduct characterization and remediation of the site.
2434943 Primary Facility 2015-12-18 Follow-up Inspection No Violations Noted Standing water in the GPU containment was measured at 4 milliSiemen/cm³. The containment is new and appears to be sound. While the Department was on-site and preparing to leave, a well tender arrived on-site and began sampling from the sand trap observation valve. The tender went to each valve, opened the valve and filled a clear quart jar. After observing the contents, the tender dumped the contents of the jar on the ground of the GPU containment (photograph 1). This is probably the reason for the elevated conductivity observed. This does not appear to be a good practice, especially when not all well pads have new containment, if any, as had been pointed out on the Charles Pad, where no containment exists(photograph 2).

2493582 Primary Facility 2016-06-13 Routine/Partial Inspection No Violations Noted The Department was on site at the Tony West (#12 Pad) to conduct a Routine Partial Inspection. At the time of the inspection, the E&S Inspection Log indicated that Klemish conducted an inspection on 6/13 and that there was no attention needed at that time. The E&S Plan map had several redlines. It was noted that the filter sock had been removed and that there were several additional topsoil stockpiles added to the site due to an excessive amount of topsoil present. The well site appeared to meet temporary stabilization requirements during the Department’s inspection.

The Department noticed that there was rust colored staining running down the production tank. The dried fluid appeared to have originated from the thief hatch on the tank. The Department requests that the operator investigate this and report back to the Department with your findings within 3 days of receipt of this report.

2540387 Primary Facility 2016-12-01 Site Restoration No Violations Noted The Department conducted a Restoration Inspection in response to the submittal of a Notice of Termination (NOT). The Department noted that the PCSM BMP did not appear to be reflective of your Site Restoration Plan. The Department spoke with Justin Thomas (SWN) concerning this and suggested that you make any repairs needed and to please contact the Department, when they have been completed, to conduct a Follow-Up Inspection. The result of the NOT is pending the Follow-Up Inspection.

2565978 Primary Facility 2017-02-28 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:30p.m.

The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, there was also a 1000psi pressure gauge with 0psi on it, the valve going to the production tank was closed. There was 763psi on the production casing and 411psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:20p.m.

2590077 Primary Facility 2017-05-01 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:10a.m.

The Department received a call about what seemed to be gas being vented at the Tonya West pad. I spoke with Field Operator Tom Leach of SWN, he told me that the 3H (115-21931) well was loaded up with fluid when the wells on the Tonya East were recently being stimulated. They were letting the pressure build up on the well and then blowing into the production tank and trying to get the fluid off the well. I contacted Steve Moyer of SWN and he said they were blowing into the production tank on April 3, 5, 10, and 13. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:30a.m.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water .5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
