2352044 Primary Facility 2015-03-10 Routine/Complete Inspection No Violations Noted I visited the Blue Beck location on March 10th 2015 at 1030am. The site was properly identified with 3 permits for the 1H, 2H, and 3H. All three permitted wells had conductors set to 40’ according to the drillers log in the mailbox by the entry sign. They were cemented in place, tagged on top of the conductor and had cattleguards in place around the cellars.
2353061 Primary Facility 2015-02-03 Preoperation Inspection No Violations Noted On 2/3/15, the Department was on site at the SWN RU 71 Blue Beck Well Pad to conduct a Preoperational Inspection. Inspection conducted with Rich Gulyas, SWN Construction Foreman. The well permits that has been issued for this pad are the Blue Beck 1H (115-21910), 2H (115-21945), and the 3H (115-21946). The site was completely snow covered, with an average snow depth of probably 2'. Did drive the access road and onto the pad. Was unable to stop and assess controls or probably would have gotten stuck. Did not find a reason not to appove the preoperational inspection, hence this inspection qualifies as a preoperational inspection. Will reinspect after snow melt.

2379117 Primary Facility 2015-06-16 Administrative/File Review No Violations Noted This administrative inspection has been created to document the small spill that was reported to the Department on 5/29/15. Attached is a copy of the spill reporting form and the complaint standard detail report. No further action will be taken by the Department at this time.

2399190 Primary Facility 2015-06-19 Routine/Complete Inspection No Violations Noted When the Department examined the liner at the joint failure location, it was noted that it had been recently patched. SWN stated that when the failure occurred, the frac fluid, while under pressure had cut the liner during the release, impacting the pad with an unknown amount of fracturing fluid. SWN stated they vacuumed up the spill then patched the liner. The Department agreed with SWN that fracturing operations could continue, and that the extent of the impact to the pad surface could be assessed after operations complete and containment removed.

The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 539psi on the production casing and 369psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:45a.m.

2684099 Primary Facility 2018-01-18 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below ice and was unable to check for gas.

I took a walk around the location and there were no obvious concerns noticed.

There were no violations noted.

2780031 Primary Facility 2018-09-26 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water 2' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2832769 Primary Facility 2019-01-28 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. The gate was open upon arrival. No one was present on pad during the inspection. Overall the site has good stabilization. No E&S issues identified. No violations noted.
