The site was last inspected by the company on 4/18/2016.

The wells associated with this site are as follows: 115-22008 115-21882 115-21888 115-22009

The site had some slippage along the access road leading in near the following GPS coordinate: 41deg. 58' 15" 75deg 37' 23". (Photos 1 and 2)

The site also had poor vegetative growth in the vicinity of coordinates: 41deg. 58' 13.8" 75deg. 37' 20.1". The poor vegetative growth could possibly be due to too much organic matter being incorporated into the soil from tree clearing operations, resulting in a tie up of the nitrogen necessary for proper germination. (Photos 3 and 4)

The sediment trap near coordinates 41deg. 58' 12.9" 75deg. 37' 21.8" is laden with sediment, most likely due to the poor vegetative growth directly uphill from the trap. (Photo 5)

2491698 Primary Facility 2016-06-06 Routine/Partial Inspection No Violations Noted The Department was on site at the Walker Diehl Well Site to conduct a Routine Partial Inspection. At the time of the inspection, the Department noted that the E&S Plan was on site and the E&S Inspection Log indicated that SRT conducted their most recent inspection on 6/6/16. The Department walked the site to verify if the recent efforts to bring the site back into compliance have worked. The Department noted a large mulch pile on the well site. Also, the site does not meet temporary stabilization requirements. During the Department’s inspection, there were large areas that had very little to no vegetation at all. The Department requests that the operator survey these areas and deploy a method of temporary stabilization consistent with your E&S Plan within 3 business days of receipt.

The Department was on site to conduct a Follow-Up Inspection of the Walker Diehl Well Site. On 6/21/16 the Department received an email from Afton Sterling (Regulatory Supervisor-SWN) concerning stabilization efforts at the NR 19- Walker Diehl Well Site. The email explained that they worked on the site that day seeding, fertilizing, and applying hay/mulch and that they will be bringing an additional 10 bales of hay and two bags of fertilizer for application the following day.

During this Follow-Up Inspection, it was noted that the well site appeared to meet temporary stabilization requirements. Areas that did not have vegetative growth consisted of mainly rock. There was a small area on the cut slope adjacent to the rock filter that could use some additional measures to acquire stabilization. There is still a large mulch pile on site. Lastly, the catch basin at the cross culvert outlet was full of water that was clear (not sediment laden as it was during the previous inspection).

2564076 Primary Facility 2017-02-23 Routine/Complete Inspection No Violations Noted Site mostly snow covered. The repairs cited in a previous inspection had been addressed and are holding up well, with visible vegetation through snow cover. The cut slope of the well pad has areas of low vegetation however this seems to be due to a local lack of good topsoil and rocky conditions. The access road is well graded and had no erosion or washouts along its length. There are no apparent erosional issues currently.

2570654 Primary Facility 2017-03-09 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:35p.m.

The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge and a 1000psi pressure gauge with about 265psi on it, the valve going to the production tank was closed. There was 608psi on the production casing and 313psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 12:50p.m.

2665816 Primary Facility 2017-11-30 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2765665 Primary Facility 2018-08-21 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water .5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
