2517550 Primary Facility 2016-09-07 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 13:15p.m., I met with company man Doug Lockwood, SDC Rig 44 was on site and drilling the hole for the production casing. The top hole section of the well was drilled and completed on 10/4/15. We took a walk around the location and noticed a small oil spot from a fuel truck and a rip in the containment, Doug was getting them taken care of while I was still there.

The depth at time of inspection was 14849'. Projected TD is 17250'. I left the site at 14:15p.m.

2541678 Primary Facility 2016-12-05 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:45a.m., USWS (US Well Service) was on site and fracking the 7H, 8H, and 9H. I met with company man Mark McPherson, they were fracking stage 31out of 49 stages on the 7H. The 9H was completed with 47 stages, and the 8H hasn’t been started with 47 stages. The water was being pumped to the site from the Odell fresh water impoundment and the Zeffer production water impoundment. The water tanks were on containment and the containment was in good condition. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 11:35a.m.

2578066 Primary Facility 2017-03-23 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection. At the time of the inspection, the E&S Inspection Checklist Site Inspection Log indicated that SRT conducted the most recent inspection on 3/17/17 and no attention needed.
2580431 Primary Facility 2017-04-06 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:15a.m.

The well was recently completed and is in production. The well tag was on the well and correct. There was a crew on site picking up the containment that was under the tanks and around the 7H, 8H, and 9H wells. The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 829psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 10:10a.m.

2637762 Primary Facility 2017-08-14 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection. At the time of arrival, I reviewed the E&S Inspection Log which indicated that the most recent inspection was conducted on 8/7/11. On site, several 21,000 gallon tanks are situated on containment. Currently the King 9H, 7H, 8H, 5H, 6H, and 3H appear to be in production, while the 1H, 2H, and 4H are present but do not appear to be on line. The access road has sediment built up in areas along the edge, in the roadside ditches, culverts, and rip rap/sump areas. Sediment has also fanned out from the access road rip rap apron/sump. Operator, please maintain the features mentioned per your approved E&S Plan.
2658342 Primary Facility 2017-10-25 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection in response to the receipt of a Post Drilling Site Restoration Report.
2706242 Primary Facility 2018-03-14 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. There were 25-500bbl water tanks and a 500bbl flow back tank on containment from a recent frac on the 1H, 2H and 4H wells, the containment was snow covered. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2809831 Primary Facility 2018-12-04 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water .5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
