Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2520140 Primary Facility 2016-09-19 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 9:10a.m. for a new spud inspection, Well Service Group was on site drilling the holes and running the conductor pipes for the 1H,4H, and 5H. The rig was drilling the mouse hole for the 1H. The drill cuttings are being put into a sealed role off container and hauled to Keystone Landfill. I took a walk around the location and everything looked OK.

The conductor pipe was set and cemented at 43' on the 1H, set and cemented at 45' on the 5H, and the hole was drilled to 45' on the 4H but they were waiting for a water truck to get the water out of the hole before running and cementing the conductor pipe. I left the site at 9:40a.m.

2531952 Primary Facility 2016-10-25 Drilling/Alteration No Violations Noted At time of inspection, I arrived on site at 11:15a.m., I met with company man Kevin Heathcock, Falcon Rig 25 was on site and just completed the fit test below the intermediate casing and was running back in the hole with drill pipe. I took a walk around the location and there were no obvious concerns noticed.

Depth at time of inspection was 1614'. Projected TD for the top hole section is 5900' I left the site at 12:10p.m.

2557397 Primary Facility 2017-01-31 Drilling/Alteration No Violations Noted At time of inspection, I arrived on site at 10:00a.m., I met with company man John Woodburn, SDC Rig 45 was on site and drilling the lateral section of the well for the production casing. The top hole section of the well was drilled and completed by Falcon Rig 25 on 10/8/16. The rig was waiting on a welder to repair a connection on the mud line at time of inspection. I took a walk around the location and there were no obvious concerns noticed.

Depth at time of inspection was 11,921'. Projected TD for the well is 13,653' I left the site at 10:50a.m.

2590072 Primary Facility 2017-05-03 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 8:25a.m.

I met with company man Mark McPherson. Halliburton was on site to stimulate the 1H,4H, and 5H wells. They completed stage 1 out of 28 stages on the 4H and were running a correlation log on the well. The 1H was completed with 27 stages. The 5H hasn’t been started yet but will have 26 stages when completed. The water was being pumped to the site from the Zeffer production water impoundment. I took a walk around the location and there were no obvious concerns noticed. I left the site at 8:55a.m.

2625643 Primary Facility 2017-08-14 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:45p.m.

The well was recently completed and is in production. The well tag was on the well and correct. The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was cement between the (20x13) and (13x9) annuli and there was no gas detected. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 508psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:30p.m.

2672120 Primary Facility 2017-12-18 Routine/Complete Inspection No Violations Noted At time of inspection.

I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2730474 Primary Facility 2018-04-25 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. No E&S issues were observed. No violations noted.
2773364 Primary Facility 2018-09-11 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water 3.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
