2520144 Primary Facility 2016-09-19 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 9:10a.m. for a new spud inspection, Well Service Group was on site drilling the holes and running the conductor pipes for the 1H,4H, and 5H. The rig was drilling the mouse hole for the 1H. The drill cuttings are being put into a sealed role off container and hauled to Keystone Landfill. I took a walk around the location and everything looked OK.

The conductor pipe was set and cemented at 43' on the 1H, set and cemented at 45' on the 5H, and the hole was drilled to 45' on the 4H but they were waiting for a water truck to get the water out of the hole before running and cementing the conductor pipe. I left the site at 9:40a.m.

2526696 Primary Facility 2016-10-11 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 11:20a.m., I met with company men Tom Fooshee, Falcon Rig 25 was on site and drilling the vertical section of the well to kick off. I took a walk around the location and there were no obvious concerns noticed.

Depth at time of inspection was 2834'. Projected TD for the top hole section is 5930' I left the site at 12:10p.m.

2590074 Primary Facility 2017-05-03 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 8:25a.m.

I met with company man Mark McPherson. Halliburton was on site to stimulate the 1H,4H, and 5H wells. They completed stage 1 out of 28 stages on the 4H and were running a correlation log on the well. The 1H was completed with 27 stages. The 5H hasn’t been started yet but will have 26 stages when completed. The water was being pumped to the site from the Zeffer production water impoundment. I took a walk around the location and there were no obvious concerns noticed. I left the site at 8:55a.m.

2625649 Primary Facility 2017-08-14 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:45p.m.

The well was recently completed and is in production. The well tag was on the well and correct. The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was cement between the (20x13) and (13x9) annuli and there was no gas detected. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 576psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:30p.m.

2677950 Primary Facility 2017-12-18 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. There was cement between the (20x13) and (13x9) annuli and there was no gas detected. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2773371 Primary Facility 2018-09-11 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water 2.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
