Minichi Contracting Group was on site and putting in the cellars. Minichi Contracting Group drilled the holes and ran the conductor pipe with a dual rotary rig on the 2H, 3H, 4H, 5H, and 6H wells. I spoke with driller Keith Bowman of Minichi, all the conductors were drilled and set at 88'. The drill cuttings were put into a sealed role off container and hauled to the Keystone Landfill. I took a walk around the location and there were no obvious concerns noticed. I left the site at 9:05a.m. |

I met with Mike Deming, Senior Regulatory Analyst and Frank Kelly Containment Coordinator both of SWNPC and Chris Anderson of Well Water Solutions. Mr. Anderson and Mr. Deming informed me the following. "Construction of the tank started yesterday. The inner tank is completed. The are (2) liners on the inner tank and (1) on the outer. Geotextile felt, (8) ounces, is on the seam of the tank walls. Geotextile felt is also on the pad surface at the bottom of the tanks, with (3) liners set in place on top of it. We still have to finish pulling the liner over the outer tank yet. The water will go from a valve cellar on the pad through a (12) inch high density polyethylene line made of plastic material and then will be pumped into the tank. The water transferred into the tank is pumped from nearby well development impoundments and the nearby Zeffer centralized wastewater impoundment through underground pipelines to the valve cellar in the corner of the pad at the base of the cut slop |

I met with company man Chris Henry. USWS (U. S. Well Services) was on site to frac the 2H, 3H, 4H, 5H, and 6H wells. They finished running the plug and perf after stage 2 and were preparing to frac stage 3 out of 24 stages on the 2H. The 3H was completed with 26 stages. The 4H was completed with 22 stages. The 5H was completed with 21 stages. The 6H was completed with 28 stages. The water was being pumped to the location from the Young fresh water impoundment. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted. |

At approximately noon today, Mike Deming, SWN Senior Regulatory Analyst contacted the Department to inform the Department that SWN had identified stormwater discharges off of secondary containment on 3 of their pads, of which this pad is one. Present during the inspection was Dillon Otten, SWN Regulatory Analyst. The Department metered for conductivity on the outside of the containment and had readings ranging from 40 uS to 255 uS. Standing water inside containment was read at 60 uS at the lowest point of containment where a discharge had occurred. 6 empty water tanks located on containment. No E&S violations were noted. No violations were observed. |
