2537812 Primary Facility 2016-11-14 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 14:00p.m. for a new spud inspection. Minichi Contracting Group was on site to drill the holes and run the conductor pipe. I spoke with driller Eric Miller of Minichi, the rig was drilling the mouse hole for the 3H. The drill cuttings are being put into a sealed role off container and hauled to Keystone Landfill.

The 3H was completed at 75', the 4H was completed at 68', and the 6H was completed at 73.5' I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:35p.m.

2543588 Primary Facility 2016-12-07 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 13:55p.m., met with company man Jeff Hill, Falcon Rig 25 was on site and drilling the top hole section of the well to kick off. I took a walk around the location and noticed sheen on the water and on the containment near the fuel tank, Jeff told me that about 4:30 a.m. on 12/7/16 there was a spill of about 8 to 10 gallons of fuel when they were filling an auxiliary fuel tank. There was no indication that any fuel got of the containment. There were no obvious concerns noticed.

Depth at time of inspection was 3,481'. Projected TD for the top hole section is 5,622'. I left the site at 15:00p.m.

2653267 Primary Facility 2017-10-27 Routine/Complete Inspection No Violations Noted At time of inspection.

Met with company man Dave Wilcox. Halliburton was on site to frac the 3H, 4H and 6H wells. They were waiting for Halliburton to repair a pump truck. They were conducting a zipper frac. There was 2 stages completed out of 39 on the 3H. There was 26 stages completed out of 36 on the 4H and 6H. The second stage sanded off on the 3H, they plan on bringing in a coiled tubing rig to flush out the sand and finish the well. The water is being pumped in from the Zeffer production water impoundment. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2694082 Primary Facility 2018-02-13 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and was in production. The well tag was on the well and correct. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2695781 Primary Facility 2018-02-14 Routine/Complete Inspection No Violations Noted Inspection conducted as a routine inspection. No E&S issues were identified. No violations noted.
2792264 Primary Facility 2018-10-22 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water .5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2826175 Primary Facility 2019-01-10 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. All 5 wells on this site appear to be in production. Overall the site has good Stabilization. No E&S issues identified. No violations noted.
