2590044 Primary Facility 2017-05-01 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 12:00p.m. for a new spud inspection.

Minichi Contracting Group was on site and putting in the cellars. Minichi Contracting Group drilled the holes and ran the conductor pipe with a dual rotary rig on the 1H, 2H, and 3H wells. I spoke with driller Keith Bowman of Minichi, the 1H and 2H wells were drilled to a depth of 140', the 3H well was drilled to a depth of 139'. The drill cuttings were put into a sealed role off container and hauled to the Keystone Landfill. I took a walk around the location and there were no obvious concerns noticed. I left the site at 12:15p.m.

2597547 Primary Facility 2017-05-24 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 13:25p.m.

I met with company men Tony Anthony. Falcon Rig 25 was on site and waiting for a generator to resume drilling. I took a walk around the location and there were no obvious concerns noticed. Depth at time of inspection was 1647'. The projected TD for the top hole section of the well is 5800'. I left the site at 14:40p.m.

2672298 Primary Facility 2017-12-18 Routine/Complete Inspection No Violations Noted At time of inspection.

I met with company man Zack Evans. USWS was on site to frac the 1H, 2H and 3H wells. The 1H was completed with 27 stages. The 3H was completed with 20 stages. They were on stage 3 out of 28 on the 2H at time of inspection. They shut down a little over half way through the stage. The manifold on the blender started leaking. The fluid was contained on the tertiary containment under the blender. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2695747 Primary Facility 2018-02-13 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. It appears as if flow back operations have been completed and that all wells are now in production. No E&S issues were noted. No violations noted.
2709600 Primary Facility 2018-03-21 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put in production. The well tag was on the well and correct. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. There were empty 500bbl water tanks on containment from the recent stimulation of the 1H, 2H and 3H wells. The majority of the containment was snow covered but what wasn't appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. The cellar was full of water and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2819991 Primary Facility 2018-12-27 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. All 4 wells appear to be in production.No E&S issues identified. Good vegetative stabilization present. No violations present.
