2591609 Primary Facility 2017-05-04 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 11:25a.m. for a new spud inspection.

Minichi Contracting Group was on site to drill and run the conductor pipe for the 1H and 2H with a dual rotary rig. I spoke with driller Ralph Dewald of Minichi, the 2H was drilled and the conductor was set at 50'. The rig was set up to start the 1H, Ralph told me they were waiting for an excavator with a jack hammer attachment because the surface is so hard. The projected TD for the 1H is 88'. The drill cuttings were put into a sealed role off container and hauled to the Keystone Landfill. I took a walk around the location and there were no obvious concerns noticed. I left the site at 12:00p.m.

2603256 Primary Facility 2017-06-12 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 8:55a.m.

I met with company man John Hill. W S G Rig #1 was on site and running the surface casing into the 1H I took a walk around the location and there were no obvious concerns noticed. I left the site at 9:50a.m.

2623840 Primary Facility 2017-08-02 Incident- Response to Accident or Event No Violations Noted I arrived on site and met with David Schoonover (DEP-OGI) and Mike Deming (SWN). The inspection was halted due to lightening, as the rig was shut down. I arrived at 1:45 PM. Mr. Deming explained the following: At 0730 on 8/1/17 the well site supervisor was conducting an inspection of the site when a few associates were washing dust from a telehandler. They were using a substance they called "rig wash." The well site supervisor discovered water on the pad when the remainder of the pad was dry. Water was pooled in an area outside of the containment. Ryan Klemish later arrived and tested some ponded water, for conductivity, on the site and found it was 2.3 mS. The well site supervisor scraped the surface of the site removing 3 buckets worth of material from the surface, which is an estimated 2-3 cu yards of material. Seth Tratthen conducted soil EC screenings after the area was excavated to determine if the release was remediated. His conductivity readings were all under 220 micro siemens. He to
2623851 Primary Facility 2017-08-03 Follow-up Inspection No Violations Noted Jon Sassi (DEP-EPS) and I met with Joseph Pendergraft (Company Man- Bedrock, SWN) and Mike Deming (Senior Regulatory Analyst-SWN). I inquired about the capacity of the power washer and Mr. Pendergraft explained that the rig wash and water is mixed at an approximate rig wash to water ratio of 1:4 in a 5 gallon bucket. They hand washed the telehadler then sprayed it down with a power washer. When I inquired about a total volume of fluid used during this practice, Mr. Pendergraft stated "I have no quantitative value that I can give you on that". Three (3) 500 bbl gallon tanks situated on Durabase Newpark mats were being drained while we were on site. They will then remove the tanks and pull back the containment. Crews were on site vacuuming up fluid on the containment, patching holes, and working on draining the tanks.

Mr. Sassi collected conductivity readings on the well pad that ranged from 0.8-3.0 mS.

2629649 Primary Facility 2017-08-22 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 14:40p.m.

I met with company man Dan Hessdorfer. SDC Rig 44 was on site and finished drilling the lateral section of the well, they were pulling the drill pipe out of the hole when I arrived on location. The top-hole section of the well was drilled and completed on 6/10/17 by WSG Rig 1 to a depth of 1630'. I took a walk around the location and there were no obvious concerns noticed. Depth at time of inspection is was 12,890'. I left the site at 15:25p.m.

2640082 Primary Facility 2017-08-29 Incident- Response to Accident or Event No Violations Noted The Department arrived on site in response to the self-reported release of a Smartwash 20/water mixture. Eric Rooney (P.G.) and I met with Mike Deming (SWN) to conduct an Incident Inspection. Upon our arrival, a vacuum truck had less than a 1 gallon release of antifreeze that they were removing from the well pad surface. Well Services Group was on site as the cleaning contractor and Resource Environmental was on site to delineate the release.

According to Mr. Deming, on 8/26/17 he received a call concerning water coming from underneath the containment on the Pease Pad. It was reported to him that they were unable to find any water on the containment, therefore they initially believed it to be water that was under the containment from condensation or a precipitation event that expressed itself while they were moving equipment off.

Mr. Deming stated that he arrived on 8/27/17 and identified the area in Photo #1 which was field tested with an approximate 2 mS conductivity. He later spoke to Steve Moy

2640102 Primary Facility 2017-08-30 Follow-up Inspection No Violations Noted The Department was on site to conduct a Follow Up Inspection, in response to a notification that SWN was removing the on site containment, of the RU 62 site. We met with Mike Deming (Senior Regulatory Analyst-SWN) Resource Environmental, their remediation contractor, found some black staining under the containment and they are collecting soil samples in bags to conduct headspace sampling. It was estimated under 5 gallons. The Department requested that they have Resource Environmental walk the fill slope to search for seeps.

Violations associated with this inspection have been attached to permit #115-22262.

2648806 Primary Facility 2017-10-18 Routine/Complete Inspection No Violations Noted At time of inspection.

I met with company man Wesley Martinez. Halliburton was on site to frac the 1H and 2H wells. The 1H was completed and the crew was changing the iron over to the 2H. The water was being pumped in from the Young fresh water impoundment. It is being pumped into a 20,000bbl tank on site. I took a walk around the location and there were no obvious concerns noticed. There were no violations noticed.

2674600 Primary Facility 2017-12-13 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and is in production. The well tag was on the well and correct. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2736020 Primary Facility 2018-05-29 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. No E&S issues were identified. No violations noted.
2768918 Primary Facility 2018-08-29 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water 3' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
