2552392 Primary Facility 2017-01-13 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 10:25a.m. for a new spud inspection. Minichi Contracting Group was on site to drill the holes and run the conductor pipe. The rig was starting to drill the hole for the 2H. The 1H and the 3H are both completed with the conductor pipe set at 88' in both wells. The drill cuttings are being put into a sealed role off container and hauled to Keystone Landfill. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 10:45a.m.

2560838 Primary Facility 2017-02-09 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 12:10p.m., I met with company man Robert Grey, Falcon Rig 25 was on site and drilling the hole for the top hole section of the well. At time of my arrival, the rig was pulling the drill pipe out the hole because of a survey tool failure. I took a walk around the location and there were no obvious concerns noticed.

Depth at time of inspection was 2185'. Projected TD for the top hole section is 5100' I left the site at 13:30p.m.

2578017 Primary Facility 2017-03-30 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 13:15p.m.

I met with company man Dan Hessdorfer. SDC Rig 45 was on site and drilling the lateral section of the well, they were in the curve at time of inspection. The top hole section of the well was drilled and completed on 2/11/17. I took a walk around the location and there were no obvious concerns noticed. Depth at time of inspection was 7,096'. Projected TD for the well is 17,729'. I left the site at 14:10p.m.

2660190 Primary Facility 2017-11-07 Routine/Complete Inspection No Violations Noted I arrived at the site at 12:18 in response to a reported spill of (15) gallons of rainwater. Mike Deming, Senior Regulatory Analyst of SWNPC informed me of the spill on 11/6/17. The weather conditions are cloudy skies, calm winds with temperatures in the mid 30's. I met with Dillon Otten, Regulatory Technician and Dan Hessdorfer, site company man, consultant, both of SWNPC. SDC #44 is the rig on the location. Mr. Hessdorfer stated the following in reference to the spill and site activities. "The Endless Mt. Rec 7H well has just been drilled to total depth at (12,848) feet. Diesel oil based mud was used during drilling when the spill occurred. The production casing will be cemented tomorrow. The rig move will start Monday. The rig will go to the Humbert (Range Unit 9) pad. The spill occurred on 11/4/17 at 6:45 AM. The trackhoe hand (operator) discovered the spill and reported it to me and then I reported it to Mr. Deming. The truck picked up the container full of drill cuttings. Rainwater between the liner in
2684048 Primary Facility 2018-01-08 Routine/Complete Inspection No Violations Noted At time of inspection.

Met with company man Eddie New. Halliburton was on site to frac the 1H, 2H, 3H and 7H wells. They were on the first stage out of 53 stages on the 2H at time of inspection. The 3H was completed with 52 stages. The 1H and 7H haven't been started, the 1H will have 56 stages and the 7H will have 23 stages when they are completed. The water is being pumped to the site from the Zeffer production water impoundment and the Young and Odell fresh water impoundments. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2685291 Primary Facility 2018-01-18 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. No leaks were detected during this inspection around the temporary water tank and associated water lines. Photo's 4 and 5 show the external water level read out attached to the water tank. No E&S issues identified during the inspection. No violations noted.
2707366 Primary Facility 2018-03-01 Incident- Response to Accident or Event No Violations Noted I arrived at the site at 12:50. The weather conditions are cloudy skies, with temperatures in the mid 40's and a light breeze. On 2/26/18 Chuck Rogers, Solid Waste Supervisor in the Waste Management Program of the PADEP Northeast Regional Office in Wilkes-Barre, PA contacted Steve Moyer, Regulatory Supervisor of SWN regarding a waste container from this pad transported to Keystone Landfill in Dunmore, PA in attempt for disposal, setting off an alarm at the landfill for Technologically Enhanced Normally Occurring Radioactive Material (TENORM) on 2/23/18. Mr. Moyer forwarded an email response in reference to the container with elevated levels of TENORM to Mr. Rogers on 2/26/18 at 9:38 AM. Mr. Moyer stated the following in the email. "Thanks for the call this morning regarding our containment waste at KSL. Some details about the origin of the load are below. Also attached is a copy of our approved OG-71b permit issued by the state. Jon Sassi was the SWS who inspected the tank prior to commissioning. The OG-71b
2717754 Primary Facility 2018-03-28 Follow-up Inspection No Violations Noted I arrived at the site at 13:20 to conduct a follow up inspection due to the discovery of a potential fluid spill at the site during an inspection conducted by the Department on 3/1/18, inspection ID #2706377. The weather conditions were cloudy skies with dense fog, a slight breeze and temperatures in the 40's. Based upon my findings at the site on 3/1/18, I planned on returning to conduct additional monitoring. I had requested that SWN conduct an investigation of the potentially impacted area on the pad surface as indicated in the 3/1/18 inspection report. For additional details pertaining to the discovery of the potential fluid spill during the 3/1/18 inspection report, refer to the inspection number mentioned above. 2729622 Primary Facility 2018-05-14 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put in production. The well tag was on the well and correct. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. There were nine 500bbl tanks on containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2830679 Primary Facility 2019-01-23 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. The cellar was full of water and there was no bubbling noticed. There was a 500bbl tank on snow covered containment, what wasn't covered appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
