Frey Well Drilling was on site to drill the hole and run the conductor pipe with a dual rotary rig. I spoke with driller Keith Bowman of Minichi, the rig was at a depth of 30'. The projected TD for the conductor pipe is 88'. The drill cuttings are being put into a sealed role off container and hauled to the Keystone Landfill. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:20p.m. |

I met with company man John Woodburn. SDC Rig #45 was on site and drilling the hole for the production casing. The top hole section of the well was drilled by WSG Rig #1 to 1643'. I took a walk around the location and there were no obvious concerns noticed. Depth at time of inspection was 5,485'. Projected TD for the well is 12,681' I left the site at 11:10a.m. |
| 2604966 | Primary Facility | 2017-06-09 | Incident- Response to Accident or Event | No Violations Noted | I met with Mike Deming, Senior Regulatory Analyst and Seth Tratthen, Regulatory Analyst of SWN shortly upon arrival to the site. Mr. Deming and Mr. Tratthen stated the following. "Southwestern Drilling Company Rig #45 is in the progress of rigging up. The company man on duty is Jonathan Woodburn. Rig up commenced on 6-7-17. The 4H well will be drilled to completion. This well was drilled to kickoff point by Well Services Group Rig # 1. SDC 45 will re-enter the well. A Caterpillar generator spilled (150) gallons of diesel fuel to containment. It is believed that the impact to the pad surface in the general area where the spill occurred is less than (10) gallons. The incident is still under investigation. A seal in the fuel transfer pump for the generator failed. It was approximately a one quarter inch weep hole that the fuel leaked through. A gasket inside of the transfer pump failed potentially due to over-pressuring or a crack. It has not been determined as to how long the pump was leaking for. There were |
