Met with company man Carl Johnson. SDC 44 was on site and drilling the lateral section of the well. They are drilling with DOBM. There are two wells in production and they are on the second well out of three to be drilled. The proposed TD for the well is 15,359'. I took a walk around the location and noticed what appeared to be hydraulic oil from a truck on the ground, Carl had a rig hand put absorbent pads down to clean it up. There were no obvious concerns noticed. There were no violations noted. |

Met with company man Derek Johnson. USWS was on site to frac the 2H, 3H and 5H wells. They just completed stage 7 out of 39 stages on the 5H when I arrived on site. There were 13 out of 45 stages completed on the 2H. There were 16 out of 49 stages completed on the 3H. The water was being pumped to the site from the Young fresh water impoundment and the Zeffer production fluid impoundment, some production water was being trucked to the site from near by well pads. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted. |
| 2774429 | Primary Facility | 2018-09-06 | Routine/Complete Inspection | No Violations Noted | Today's inspection was conducted as a small spill complaint inspection after Mike Deming, SWN Senior Regulatory Analyst contacted the Department on 9/5/18 at approximately 3:05 PM to report that on September 5, SWN Regulatory was notified by SWN Production of an apparent dark stain with associated white crystallization on the Buckhorn pad surface after the ground near the wellhead, where secondary containment was removed, began to dry in the sun. After further visual observations were made by regulatory, it was determined the release was likely greater than five (5) gallons off containment and the Department was immediately notified. It was reported as an area approximately 30’ feet x 30’ feet, and likely caused by a small unnoticed compromise in secondary containment in or near the stained area that allowed some fluids to escape during completions operations.

The well was recently completed and put in production. The permit numbers on the entrance sign were correct. The well tag was on the well and correct. There was containment around the well and made inspection of the cellar limited. The cellar was set up with a plastic corrugated pipe around the well for inspection access. The cellar was full of water and there was no bubbling noticed. There were 23 empty 500bbl tanks on containment from a recent frac of the 2H, 3H and 5H wells, there was snow and ice on the containment making inspection of the containment limited. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted. |
