On site for a new spud inspection. Met with Minichi Contracting driller Keith Bowman. The 1H and 3H were both drilled with a dual rotary rig to a depth of 88'. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2727044 Primary Facility 2018-05-07 Drilling/Alteration No Violations Noted At time of inspection.

Met with company man Johnny Hale. Falcon 24 was rigged up on the 3H and running the surface casing. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2734788 Primary Facility 2018-05-29 Drilling/Alteration No Violations Noted At time of inspection.

Met with company man Carl Johnson. SDC 44 was on site and drilling the lateral section of the well. The top hole section of the well was drilled and completed on 5/15/18 by Falcon 24. Proposed TD for the well is 13,137'. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2744509 Primary Facility 2018-06-22 Routine/Complete Inspection No Violations Noted At time of inspection.

Met with company men Johnathon Sites and Mike Tornes. USWS was on site to frac the 1H and 3H wells. They were fracturing stage 6 out of 23 on the 1H and running a plug and perf in the 3H when I arrived. There were 7 stages completed out of 27 on the 3H. The water is being pumped to the location from the Young and Odel fresh water impoundments and the Zeffer production fluid impoundment. Production water from nearby well pads are being trucked to the location. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2747711 Primary Facility 2018-06-28 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a complaint inspection. SWN notified the Department after hours and reported that a heavy rain event overnight overwhelmed their secondary containment of the existing tank battery. SWN estimated that approximately 75 to 100 barrels of water was released when the containment failed. Today's inspection found that the water released did not indicate that it was contaminated and that there was no sheen present. Photo's 2, 3, and 4 show the area that failed and was immediately repaired. As photo's 3 and 4 show, he release did not cause damage in the form of accelerated erosion. Also present during the inspection was Frank Hoehle, Department WQS and Mike Tornes, Site Supervisor. Fracking operations have been completed. Plan is to use the existing secondary containment for drill out operations which are scheduled to take place in about one week. No violations noted.
2776739 Primary Facility 2018-09-19 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put in production. The well tag was on the well and correct. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water 3.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. There were five empty 500bbl water tanks on containment from a recent stimulation and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2830238 Primary Facility 2019-01-22 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. As the photo's show, the site is snow covered. No obvious stabilization problems were evident. No E&S issues identified. No violations noted.
