2668510 Primary Facility 2017-12-07 Drilling/Alteration No Violations Noted At time of inspection.

On site for a new spud inspection. Minichi Contracting Group was on site to drill and set the conductor pipe for the Conigliaro 1H and 2H. I met with the driller Ralph Dewald. The rig was on the 2H and at a depth of 68'. The 1H was drilled and the conductor pipe was set at 88'. They were drilling with a dual rotary rig. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2670068 Primary Facility 2017-12-11 Routine/Complete Inspection No Violations Noted I inspected the Range Unit 5 well site on December 11, 2017. The site was solid and stable, as was the access road. There was pipeline work occurring next to the access road. The work was temporarily stabilized. The wells associated with this site were the

There was adequate vegetation on the cut and fill slopes, the pad surface was stable and there were no erosional issues apparent at the time of inspection.

There were no violations noted.

2690342 Primary Facility 2018-02-02 Drilling/Alteration No Violations Noted At time of inspection.

Met with company man Tom Howard. SDC44 was on site and drilling the hole for the production pipe. They were still in the vertical section of the well and still drilling on air. The proposed TD for the well is 12,637. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2699338 Primary Facility 2018-02-28 Routine/Complete Inspection No Violations Noted At time of inspection.

Met with company man Jonathon Sites. U S Well Service was on site to frac the Conigliaro 1H and 2H wells. They were on stage 8 out 56 stages on the 2H. They completed stage 8 out of 29 stages on the 1H and was running the plug and perf. The water was being pumped to a Mass ( Modular Aboveground Storage Structure) on location from the Young and Odel fresh water impoundments I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2703066 Primary Facility 2018-01-29 Incident- Response to Accident or Event No Violations Noted I arrived at the site at 10:44 in response to a spill of (10) gallons of freshwater mixed with cement. The weather conditions were partly cloudy skies with a light breeze, and temperatures in the mid 30's. SWN Rig #44 is the drilling rig on the pad. The current drilling depth is (1,620) feet. I met with Mike Deming, Senior Regulatory Analyst and Tommy Howard, site company man/well site supervisor, both of SWN. Mr. Deming and Mr. Howard provided me the following information regarding site activities and the spill. "We are drilling the Conigliaro 1H well. Halliburton is getting ready to cement the intermediate casing at (1,620) feet. The surface casing was cemented at (580) feet. Kickoff point (KOP) is at (5,150) feet. The well will be drilled to total depth at (12,637) feet. We will air drill the well to KOP and drill with diesel oil based mud from KOP to total depth. The surface section was air/mist drilled. The Conigliaro 2H well has been drilled to total depth. This rig can drill wells from top to bottom.
2714256 Primary Facility 2018-02-28 Routine/Complete Inspection No Violations Noted I arrived at the site at 10:23. David Schoonover, DEP OGI was also on-site conducting an inspection. The weather conditions were sunny skies, a light breezy and mild with temperatures in the 50's. US Well Service is the SWN subcontractor at the site in the process of conducting hydraulic fracturing (stimulation) operations. SWN submitted a Stimulation notification per 58 Pa. C.S. 3211(f)(2) on 2/12/18. The notice indicated that stimulation of the Conigliaro 1H and 2H would commence on 2/22/18 at 6 AM. I met Jonathan Sites, well site supervisor/site company man, Jonathan Davidson, Senior Workover Completions Foreman and Mike Deming, Senior Regulatory Analyst all of SWN. Mr. Sites and Mr. Deming stated the following in reference to site activities and the modular aboveground storage structure (MASS) as per the OG-71 Form B application accepted by the Department on 2/14/18. "Zipper frac is in progress on the Conligliaro 1H and 2H wells. The frac job started on 2/25/18. We are on Stage 8 of 29 on the 1H and Stag
2736298 Primary Facility 2018-05-31 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put in production. The well tag was on the well and correct. The cellar was set up with a corrugated pipe around the well for inspection access. There was containment around the well, I pulled back the containment at the base of the well and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2763023 Primary Facility 2018-08-13 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water 3.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
