Minichi Contracting Group was on site to drill and set the conductor pipe on the 1H and 2H wells. I met with Minichi driller Keith Bowman. The 1H was completed at a depth of 88'. They were at 60' on the the 2H and plan to TD at 88'. The drill cuttings are put in roll off containers and hauled to the Keystone landfill. I took a walk around the location and there were no obvious concrns noticed. There were no violations noted.

2760280 Primary Facility 2018-07-30 Routine/Complete Inspection No Violations Noted I inspected the Mitchell well pad on 07/30/2018 in response to a spill notification.

I met with company man Tommy Howard on site to perform an inspection. There apparently had been communication between the well bore and the mousehole which was discovered when a righand stepped on the plastic surrounding the hole and discovered it was puffed up with water. The water went back down shortly thereafter and there was no residual water after that initial discovery. The company had just started pumping cement and was just getting returns when it was discovered. The company is currently drilling on the Mitchell 1h. When the incident was noticed they had been running 13 3/8" surface casing which was just finished being flushed out with fresh water.


When the rig moves off, the site will be checked out after the plastic is removed and screened for contamination according to the company man.

The cut and fill slopes are well vegetated and stable.

2760744 Primary Facility 2018-08-06 Drilling/Alteration No Violations Noted At time of inspection.

Met with company man Tommy Howard. SDC 44 was on site and drilling the vertical section of the well. They are drilling with air to 5650'. There are four existing wells on site that are still in production. There are two new wells to be drilled, they are on the first of the two wells. The proposed TD for the well is 17547'. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2783754 Primary Facility 2018-10-03 Routine/Complete Inspection No Violations Noted At time of inspection.

Met with company man Aron Lash. USWS was on site to frac the 1H and 2H wells. The were running a plug and perf in the 1H and stimulating stage 15 on the 2H when I arrived on location. There were 15 out of 75 stages completed on the 1H. There were 14 out of 94 stages completed on the 2H. The water was be pumped from the Odell fresh water impoundment to the NR3 pad then trucked to the HDK pad, from there it was being pumped to the frac site and production water was being trucked directly to the frac site. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2837791 Primary Facility 2019-02-08 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put in production. The permit numbers on the entrance sign were correct. The well tag was on the well and correct. There was containment around the well and made inspection of the cellar limited. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water 2' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
