On site for a new spud inspection. Met with Minichi driller Keith Bowman. Minichi Contracting Group was on site to drill and run the conductor pipe in the 1H, 2H, 5H and 6H wells. There are two existing wells on site, the 3H and 4H that are in production. They were on the 1H at a depth of 155', Keith said they should TD at 215'. The conductor pipe on the 3H and 4H were set at 215'. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted. |

Met with company man Johnny Hale. Falcon 24 was on site to drill and run the surface and intermediate casings. They were moving the rig onto the 2H when I arrived on site. There are two wells on the pad that are in production and four new wells to be drilled. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted. |
| 2850984 | Primary Facility | 2019-02-19 | Incident- Response to Accident or Event | No Violations Noted | I arrived onsite at 1400 hours to conduct an incident response inspection after receiving a spill report stating that 30 gallons of fresh water mixed with fine sediments from drill cuttings had been inadvertently released from a water truck and impacted a portion of the pad's access road. The weather was sunny and cold at 25 degrees F with calm winds and no precipitation. Drilling operations were underway utilizing SWN's Rig 45.
Upon my arrival, I entered the company man trailer to check in. No one was inside, so I asked several nearby workers where I could find him. One of them stated that he believed the company man was on the rig floor. Since I did not want to go up there unaccompanied, I asked the workers to radio him for me. They were not equipped with radios, but one of them offered to walk up to the rig floor and inform him I was there for an inspection. After several minutes, the company man eventually walked down and introduced himself as Ben Dodd. As Mr. Dodd and I entered his trailer, I informed |
