2626560 Primary Facility 2017-08-09 Routine/Partial Inspection No Violations Noted On 8/9/17 at 1405 hrs I conducted an inspection of the Thomas Unit 1 well site. No one was present during the inspection. It was mostly sunny and 78 degrees F.

I observed the site and reviewed the erosion and sedimentation (E&S) routine inspection log. The site consists of the access road and well pad. The Unit 1 well is a vertical Marcellus well that was drilled by Ultra Resources in 2008.

SWEPI submitted a Well Site Restoration Report (WSRR) on 7/25/17 for the Thomas 808 well site; the Unit 1 well was also included on the report. A review of Department records shows that Ultra Resources previously submitted a WSRR for the Unit 1 well; it was received and acknowledged by the Department on 4/20/09.

The well site is restored and permanently stabilized as per 25 Pa Code Section 102.22(a)(2). The last routine E&S inspection was conducted on 6/15/17.

The well pad appears to be a staging area for various supplies and equipment. Of note, numerous empty totes were stored at the site. Items not needed for produ
