The Marshlands Unit 3, 4H and 6H wells have been drilled and are in production and 2H well has a permit, but has not yet been drilled. During this inspection the ground was partially frozen. The perimeter of the site was walked and all erosion control BMPs were observed. There was no evidence of any spills found. The cut slopes of the access road are in need of stabiliazation. These areas must be covered with either a minimum uniform coverage of mulch and seed, with a density capable of resisting accelerated erosion and sedimentation or an acceptable BMP which minimizes accelerated erosion and sedimentation. No violations were cited during this inspection. - MRN
2461794 Primary Facility 2016-03-02 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Kjelgaard 802 site located in Gaines Twp, Tioga Co. on 3/2/2016 @1230 hours. The weather was 25° cloudy/snow flurries with 28 mph winds out of the west. Copies of the well permits were mounted on the backside of the site ID sign. The onsite inspection log was located in the mailbox. The site was last inspected by company personnel on 2/23/2016 with “no visible issues” recorded in the comments section.

Three wells were observed on the pad surface, the Marshlands Unit 3, 4H and 6H. Well tags were observed on the well heads. The secondary containment surrounding the produced water tanks was holding frozen water. The production equipment for these wells is located on a lower pad area along the access road. All rock lined ditches (RLDs) appeared to be functional. Cut slopes and fill slopes appeared well vegetated. No violations are issued at this time. Please contact me with any questions. 2521555 Primary Facility 2016-09-14 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Kjelgaard 802 site located in Gaines Twp, Tioga Co. on 9/14/2016 @ 1300 hours. The weather was 75° and sunny. Copies of the well permits were mounted on the backside of the site ID sign. The onsite inspection log was located in the mailbox. The site was last inspected by company personnel on 8/29/2016 with “no change” recorded in the comments section.

Three wells were observed on the pad surface, the Marshlands Unit 3, 4H and 6H. Well tags were observed on the well heads. The production equipment for these wells is located on a lower pad area along the access road. All rock lined ditches (RLDs) appeared to be functional. The newly constructed infiltration berm appears to be stabilized with vegetation. Residual waste containers are located on containment on the pad surface. 2536436 Primary Facility 2016-11-08 Site Restoration No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Kjelgaard 802 site located in Gaines Twp, Tioga Co. on 11/8/2016 @1400 hours, in response to receipt of the Notice of Termination by the DEP on 10/19/2016. Copies of the well permits were mounted on the backside of the site ID sign. The onsite inspection log was located in the mailbox. The site was last inspected by company personnel on 10/31/2016 with “no visible issues” recorded in the comments section.

Three wells were observed on the pad surface, the Marshlands Unit 3, 4H and 6H. Well tags were observed on the well heads. Two waste containers are observed on containment on the pad surface. An infiltration berm was observed below the fill slope on the south side of the pad. Sediment traps on the west side of the pad have also been converted to infiltration berms. 2659015 Primary Facility 2017-11-09 Routine/Complete Inspection No Violations Noted Routine inspection of three unconventional wells in production. All three wells had a production tree with plunger lift. There were digital pressure gauges on wing valves for production casing pressure. The backside of production casing was open and venting. The cellars were full of gravel. No obvious issues were observed.

2791030 Primary Facility 2018-10-18 Routine/Complete Inspection No Violations Noted The DEP conducted a ROUTINE inspection of the SWEPI LP - Kjelgaard 802 well pad located in Gaines Twp, Tioga Co. on 10/18/2018 @ 1300 hours. The weather was 37 degrees, partly cloudy, 65% humidity, winds SW @12 mph. No activity was observed onsite.

The MU3, 4H and 6H wells were observed in production onsite. The pad has been reduced, and the surrounding area is stabilized by >70% vegetative cover. The ESCGP was NOT'd in November of 2017. Some standing water was observed on the pad surface, all conductivities measured were in normal range. No erosion or sedimentation was observed. No violations are noted at this time.
