2048179 Primary Facility 2012-03-05 Routine/Complete Inspection No Violations Noted A slope at the rear of the site was slumping and is need of re-grading and stabilization. Several areas, including the fill slopes of the water impoundment are not stabilized. These areas must be covered with either a minimum uniform coverage of mulch and seed, with a density capable of resisting accelerated erosion and sedimentation or an acceptable BMP which minimizes accelerated erosion and sedimentation. Elevated readings for specific conductivity (approximately 2 mS) were measured in puddles on the northeast side of the pad approximately 100 yards from the road. This should be investigated. A follow-up inspection will be conducted to determine if escalated enforcement is necessary. The pipeline and compressor station were not inspected at this time. No violations were cited during this inspection. Violations for 25 PA code §102.22(b) noted during a April 6, 2011 inspection remain outstanding against the 6H well, all other permits associated with this site will be recorded as NOVIO. - MRN

The date of the last inspection conducted by AMEC is unknown. 25 PA Code §102.4(b)(5)(x) requires a written report documenting each inspection, this should include the date the inspection was conducted. The site is currently being restored. The area has been restored to near original contours and the water impoundment is no longer present. The E&S controls were installed and functioning. The site needs to be stabilized as soon as possible to reduce the risk of accelerated erosion. The Pipeline and Compressor Station were not part of this inspection. The photos are attached to the ESX10-117-0013 inpsection report. No violations were cited during this inspection. - MRN

The weather was 30 D, cloudy with snow squalls. A light dusting of snow was observed on the pad surface. The site appeared stabilized. No E&S issues were observed. 2714789 Primary Facility 2018-04-02 Site Restoration No Violations Noted The Department of Environmental Protection inspected the SWEPI LP – Lick Run 803 well site on April 2, 2018 @ 1245 hours. The site ESCGP included the well pad, compressor station and PL-106 (connecting the well pad to the compressor station). The weather was 42 degrees and sunny. There was no activity onsite during my inspection.

The 1H – 6H wells are drilled and in production. The well pad has been restored and PCSM BMPs have been installed. The NOT and WSRR were approved on 7/27/2017. The pipeline and well pad appeared to be stabilized with a permanent vegetative cover during the inspection. Vegetated portions of the pad appeared to be saturated with water and have poor drainage.

No violations are noted at this time. I departed the site at 1330 hours.
