2782226 Primary Facility 2018-09-21 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of the SWEPI Clark 486 Well Pad in Sullivan Twp, Tioga County on 9/21/2018 at 1320 hours. The inspection was conducted in response to a Notice of Termination received by the DEP. The weather was 67 degrees, cloudy, and 88% humidity.

There are currently 3 producing wells onsite and another well currently inactive. There is 1 highland production tank onsite. The compressor pad is just southwest of the wells (see photo #4). All PCSM BMPs appear to be in place. The pad appears to be well vegetated and only minor erosional issues were noted on the lease road. The small stretch of sock that was noted has now been removed, so no temporary E&S BMPs remain onsite. A post drilling well site restoration report has been accepted by the DEP.

This Notice of Termination is recommended for approval. No violations noted at this time.
