AMEC conducted an inspection of the site on behalf of Ultra on Mar. 7, 2012 and noted that the vegetative growth for the infiltration berms needs to be monitored in spring 2012. Multiple areas of the cut and fill slopes of the pad, the pipeline along the access road and the topsoil stockpiles are in need of further stabilization. These areas were either not seeded or the seed did not germinate. Please evaluate the area to determine what further action is required. - MRN

2167025 Primary Facility 2013-05-14 Routine/Partial Inspection No Violations Noted An routine partial inspection of the Tract 846 - 822 pad in Gaines Township, Tioga County was conducted on May 14, 2013. There are six drill permits associated with this site. The 1H, 2H and 3H wells are on active status, the 4H, 5H and 6H wells have been reported not drilled by the operator . The 1H and 2H wells have been drilled, but are not in production. The 3H well is permitted, but has not been drilled. The pad surface is stabilized with rock. The slopes were covered with erosion control matting at one time, but matting has since deteriorated. Multiple areas of the cut and fill slopes of the pad, the pipeline along the access road and the topsoil stockpiles are in need of further stabilization. These areas were either not seeded or the seed did not germinate. Please evaluate the area to determine what further action is required. No puddles exhibiting elevated conductivity or erosion issues were observed at this time. No violations were cited during this inspection.
2206241 Primary Facility 2013-09-23 Routine/Partial Inspection No Violations Noted On September 17, 2013 an inspection was conducted at the State Tract 856 822 Pad in Gaines Township, Tioga County. The last inspection conducted on behalf of the operator was on 9/12/13, the only issue noted was that previously noted issues have not been addressed. During this inspection the following was noted: the cut slope below the pad are recently seeded and mulched. This site is in need of additional E&S controls until 70% vegetative stabilization is established. A copy of the E&S plan was not maintained on site.
2477764 Primary Facility 2016-04-21 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) conducted a routine partial inspection of the SWEPI LP – DCNR Tract 856 822 Well site (ESG10-117-0062) on 4/21/2016 @1230 hours, located in Gaines Twp., Tioga Co., during an onsite meeting to discuss restoration of the site. Present for the meeting were Dan DiCamillo, DCNR Forester & Gas Lease Administrator; Jason VanDergrift, SWEPI – Civil Construction Supervisor; and a representative from contractor, J.L. Watts. The State 822 well pad is located in the Painter Run and an UNT to Right Branch watersheds. Painter Run is a tributary of Pine Creek and is classified as a HQ, with protected use for CWF and MF. Right Branch is a tributary to Fourmile Run. Right Branch is not listed in Chapter 93. Fourmile Run is designated as HQ, with protected use for CWF and MF. The site was last inspected by company personnel on 4/14/16 with “No visible issues” noted in the comment section. The E&S plan and PPC plan were present in the mailbox onsite.

2481778 Primary Facility 2016-05-05 Routine/Complete Inspection No Violations Noted
2604845 Primary Facility 2017-06-12 Site Restoration No Violations Noted On 6/12/17 at 1400 hrs I conducted a restoration inspection of the State 822 well site. No one was present during the inspection. It was mostly sunny and 82 degrees F.

I observed the site and reviewed the site restoration drawing, the erosion and sediment (E&S) inspection log, the Notice of Termination (NOT) and the Well Site Restoration Report (WSRR). The NOT and WSRR were received on 6/6/17.

Post-construction stormwater management (PCSM) BMPs were installed at the site in May 2016 (Photos #1 & 4). Temporary E&S controls have been removed and the site is permanently stabilized as per 25 Pa Code Section 102.22(a)(2).

The well pad has not been minimized (Photo #2). On 6/15/17 I spoke with Tim Smith, SWEPI Senior Environmental Regulatory Engineer, regarding this and other site restoration questions. Mr. Smith stated that the State 822 pad was not minimized as part of the restoration work because it is SWEPI's intent to drill at the site within the next several years. Based on SWEPI's intent to drill, the

2608742 Primary Facility 2017-06-29 Administrative/File Review No Violations Noted On 6/29/17 I reviewed the supplemental information provided by SWEPI for the State 822 well site and the PL-122 pipeline project; both projects were permitted under the same ESCGP.

The Notice of Termination (NOT) and the Well Site Restoration Report (WSRR) were received on 6/6/17. A site inspection was conducted on 6/12/17 to validate restoration activites.

As documented in the 6/12/17 inspection report, the site met the restoration requirements, however, additional documentation was needed regarding the PL-122 portion of the ESCGP.

On 6/28/17, SWEPI provided an aerial map and plan drawing, showing that the PL-122 project was never constructed. The PL-122 pipeline was to connect the PL-121 pipeline to the State 822 well pad.

Based on the supplemental information provided, I recommend approval of the NOT; the WSRR for the 1H and 2H wells was acknowledged on 6/29/17.

No violations are cited as a result of this administrative review. TLT
