2531679 Primary Facility 2016-10-26 Routine/Complete Inspection No Violations Noted Inspection of EQT’s Phoenix 590796 (117-20970) unconventional gas well in Duncan Twp. Tioga County conducted October 26, 2016 at 11:50 am. At the time of inspection; the well had a production tree in place, shut-in. Analog pressure gauge on the tree top was shut-in from well, 0 PSI. There was a digital scada gauge on the production casing wing valve down in the cellar; not visible. The valve for the backside of the production casing (9 5/8” x 5 ½”) annulus was partially buried at the bottom of the cellar. The valve was open at the wellhead but shut in at the vent pipe plumped to surface. The operator has plans to plug this well. There are (6) other active wells on the pad in production; #590794 (117-20969), #590792 (117-20980), #590791 (117-20967), #590790 (117-20965), #590627 (117-20725), and #590025 (117-20994). Visual inspection of WH and cellars; no obvious concerns. Went over plugging procedure for the -796 with the operator. Expected to rig up on the well next week for plugging ops.
2533190 Primary Facility 2016-11-01 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of EQT’s Phoenix 590796 (117-20970) unconventional gas well in Duncan Twp. Tioga County conducted November 1, 2016 at 10:00 am. At the time of inspection; a workover rig was rigged up on the well for plugging ops. I met with the Companymen to discuss operation on the pad. Wireline was rigged up for gauge run down to the obstruction (6470’). If run is successful they’ll set the CIBP in the 5 ½” prod. csg. @ 6450’, cap with cement using dump bailer. Tomorrow they’ll circulate gel, set second CIBP (4070’), run freepoint tool and shoot off casing above freepoint (pull ~3626’ 5 ½” production casing). Allied is lined up to mix, pump, and spot (3) balanced cement plugs; #1) from the production casing stub (~3626-2526) in the 8 ½” open hole, #2) in the open hole, across the 9 5/8” shoe up into the 9 5/8” (2576-2126), and #3) (1275-1225, in the 9 5/8” csg). I plan to return Thursday when they estimate spotting balanced plugs and asked the Companymen to catch a slurry sample if I’m not on location. Aside fr
2534417 Primary Facility 2016-11-03 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of EQT’s Phoenix 590796 (117-20970) unconventional gas well in Duncan Twp. Tioga County conducted November 3, 2016 at 8 am. At the time of inspection; a workover rig was rigged up on the well for plugging ops. I met with the Companymen to discuss operation on the pad since my last visit Tuesday. CIBP has been set, 6450’ with 15’ cement dump bailed; sample displayed good strength. The wireline perf’d the 4 ½” at 3850’ so the rig can establish circulation to flush mud out of the annulus to avoid the well U-tubing when they go to pull casing (heavy drilling mud on the backside of csg), and ensure casing is free. The rig couldn’t establish circulation here; went uphole w/ perfs (3650’, 3450’, 3250’) and broke circulation at 3052’.. Plans going forward; shoot & pull production casing, cement w/ Allied as scheduled for remaining balanced cmt plugs. 2535454 Primary Facility 2016-11-07 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of EQT’s Phoenix 590796 (117-20970) unconventional gas well in Duncan Twp. Tioga County conducted November 11, 2016 at 12 pm. At the time of inspection a dump truck was unloading gravel in the 9 5/8” on top of final cement plug to top off casing to surface. I spoke will the Companymen to discuss operations on the pad since my last visit. First attempt to shoot casing collar @ 3052’ failed, moved uphole 2 joints & shot csg collar at 2974. The rig pulled 2974’ of 5 ½” csg and set remaining 3 balanced plugs; 1 in the open hole, 1 across the 9 5/8” shoe and 1 in the 9 5/8”. All 3 slurry samples looked good, set up and displaying good strength. Waiting on Plug Cert, No issues.
2540329 Primary Facility 2016-11-23 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of EQT’s Phoenix 590796 (117-20970) unconventional gas well in Duncan Twp. Tioga County conducted 11/23/16 at 1 pm. Rig on location in effort to remediate reported bubbling inside the 9 5/8” several days after rigging down. Well reportedly builds ~30-75 PSI after shutting in overnight (~12hrs). The valve for backside of 9 5/8” is open to plastic bag which reportedly does not inflate when the 9 5/8” is shut in, or open. At the time of inspection; the valve for the 9 5/8”s annulus was underwater in the cellar with no observed bubbling. There was bubbling inside the 9 5/8”. Operator’s tentative plans are to remove gravel, drillout surface plug to tag 9 5/8” plug and set CIBP on the 9 5/8” shoe plug (~2576’-2126’), top with cement. I advised to only proceed with this as long as there is good integrity at the 9 5/8” shoe and annulus and recommended to shut the well in for a 24 hr period to make sure the well does not build excessive shut in pressure in contrast with previous 12 hr SI and more important
2540409 Primary Facility 2016-11-30 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of EQT’s Phoenix 590796 (117-20970) unconventional gas well in Duncan Twp. Tioga County conducted 11/30/2016 at 2:30 pm. WO Rig on location for remediation after bubbling inside the 9 5/8” was noticed after plugging ops. The rig just finished shut in test per my recommendation to verify annular integrity on the 9 5/8” x 13 3/8” annulus and to verify pressure build up is not linear to point of overpressuring the 9 5/8” csg shoe and will begin to level off at some point, before going ahead and approving any remedial procedure. 2545945 Primary Facility 2016-12-13 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of EQT’s Phoenix 590796 (117-20970) unconventional gas well in Duncan Twp. Tioga County conducted 12/13/16 at 11 am. WO Rig on location for remediation after bubbling inside the 9 5/8” was noticed after plugging ops. I met with the Companyman to discuss operations since my last visit 11/30. The rig drilled out (2) cmt plugs til they found good cmt @ ~2219. They set (2) CIBPs above that point, dump bailed 5’ cmt on top, some bubbling still reported at that point. They ran a downhole camera for which I reviewed the video on location; bubbling was definitely coming from below the bridge plugs, csg collars looked OK. At that point they circulated the hole and reportedly had no bubbling since. They set (2) more balanced cement plugs in the 9 5/8” as a precaution. The last blend they ran was 10% salt w 2% calcium chloride; samples showed good strength. No integrity issues reported on the 9 5/8”s annulus. Aside from the well being plugged there are (6) active wells on the pad in production; #590794 (117-
2546449 Primary Facility 2016-12-21 Plugging(Includes Plugged/Mined Through) Pending Inspection of EQT’s Phoenix 590790 (117-20970) well conducted December 21, 2016 at 1 pm. The rig has rigged down and pulled off the pad following plugging operations. At the time of inspection the well was capped with a 3” vent pipe about 7’ above the top of the grated cellar tee’d at the top. A field reading at the vent pipe with an Altair 5x gas meter yielded 100% LEL and 82% methane. I notified the operator about my findings and discussed. I requested the operator submit a plugging certificate as normal and I would re-inspect in 30 days before approving or denying. If gas was still present I would likely re-inspect after an additional 30 days to see if the gas is decreasing, persistent, or increasing. From that point a determination would be made how to proceed or if further work may be necessary. “Violations Pending” to 25 PA Code Ch. 78.91 (Plugging) Pending results to follow up inspections.
2551762 Primary Facility 2017-01-09 Routine/Partial Inspection No Violations Noted Routine partial inspection. Site snow covered. Well pad being prepared for arrival of the drill rig. Five new wells permitted and were reported as spud on 12/27/16. Entire pad surface covered by mats, which were underlain with textured liner. Crew on site working to berm up the liner around the perimeter of the pad. Impoundment 2 was frozen over and snow covered; adequate freeboard remained. Multiple piles of spoil material observed at staging area along access road. 2555002 Primary Facility 2017-01-25 Plugging(Includes Plugged/Mined Through) Pending Inspection of EQT’s Phoenix 590790 (117-20970) well conducted January 25, 2017 at approximately 9:30 am. There was a spudder rig on location to drill tophole section of newly spud wells. I met with the Companyman to direct me to the vent where the 590790 was venting to the edge of the pad. The vent was about 8’ or more high so I was unable to access the top of the vent. The 2” vent standpipe had a ½” needle valve with a (0-2000PSI) analog pressure gauge in line. I shut in the 2” ball valve downstream of the pressure gauge to isolate the gauge from the open vent. I waited ~5 minutes and did not observe any pressure build on the gauge. I shut in the needle valve, removed the (0-2000PSI) analog gauge and installed my (0-160PSI) pressure gauge, then reopened needle valve to gauge. There was no pressure observed. I opened the ¼” bleeder line off my pressure gauge and did not observe any flow. I removed my pressure gauge and installed a 1/2” pipe nipple w/pex cap and hose hooked directly to an Altair 5x gas meter
2561945 Primary Facility 2017-02-15 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of EQT’s Phoenix 590790 (117-20970) now plugged well conducted February 15, 2017. I arrived on location at approximately 10 am. There was a spudder rig on location for tophole sections of newly spud wells. I met with the Companyman and went to the edge of the pad to the vent pipe for the the 590790. The 2” vent standpipe had a ½” needle valve with a (0-2000PSI) analog pressure gauge in line. I shut in the 2” ball valve downstream of the pressure gauge to isolate the gauge from the open vent. I waited ~5 minutes and did not observe any pressure build on the gauge. I shut in the needle valve, removed the (0-2000PSI) analog gauge and installed my (0-160PSI) pressure gauge, then reopened needle valve to gauge. There was no pressure observed after waiting 5 minutes. I removed the gauge and threaded a ½” pipe nipple with balloon attached, waited 5 minutes. There was zero inflation of the balloon indicating no measurable flow. The well meets the plugging requirements of 25 PA Code Ch.78.91 with regard to
2592179 Primary Facility 2017-05-10 Routine/Complete Inspection No Violations Noted I arrived on-site @11:20 hrs and met Bill Wallace of SWN. Patterson/UTI rig 294 on site, running pipe into the 593939 well, and anticipate skidding the rig to the 593940 well tonight. Site was neat, secondary containment was present and in good condition on top of rig mats. Chemical storage and backyard looked good. I departed the site @11:50 hrs.
2614014 Primary Facility 2017-07-06 Routine/Partial Inspection No Violations Noted No activity on the pad when I arrived. 10 K bbl. tank had been erected on the site, approximately 1/2 full of what appeared to be precipitation. Some water had collected in the pad containment near the edges; conductivity of this water was in the .45- .67 mS/cm range. The tank also appeared to have a remote monitoring system hooked up to it. Rig mats were still down over the entire pad from drilling operations. The liner berm at the site entrance seemed somewhat worn; I recommend that it be fully assessed before any fracing operations begin.
2657498 Primary Facility 2017-11-01 Incident- Response to Accident or Event No Violations Noted I met with Corey Giles of EQT and Neil Ketchum of Groundwater Sciences when I arrived. My inspection was in response to a reported release to the drainage ditch on the south side of the pad. A third party EHS group discovered water with elevated conductance (1-3.5 mS/cm) in the ditch on 10/31. The area had been more or less continuously vacced between the time it was discovered and the time I arrived. Corey and Neil had just finished taking soil and water conductance readings along the length of the ditch (see attached photos). Their investigation showed a gradient, beginning on the south side of the pad in the ditch approximately 100-125' east of the pad entrance, with the highest soil and water conductance in that area (6.77 mS/cm water, 2.57 mS/cm soil) and declining readings as they probed downgradient. At the time, the source of the water was unknown, but the area where the water was coming into the ditch seemed to be at the area of highest conductance. The containment berm on the pad had not yet
2674555 Primary Facility 2017-12-26 Plugging(Includes Plugged/Mined Through) No Violations Noted Routine Inspection of a plugged well. The temporary vent to the edge of the pad has been rigged down and a 7-8' permanent marker with vent is in place. No visual or audible flow was observed. The vent was about 2" out of my reach, still no gas detected at this point though. No obvious issues observed.
2714972 Primary Facility 2018-04-03 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of this plugged unconventional gas well was conducted April 3, 2018. At the time of inspection, the plugged well has a cap down in an open cellar with metal grating across it at ground level. There is a 2" wellbore vent pipe coming out of the cap which extends through the grating to approximately 8' above ground level. There was no visual or audible flow coming from the vent. No gas was detected with an Altair 5x gas meter at the vent. The 9 5/8" casing, once vented to the edge of the pad, is now shut in to an analog pressure gauge down in the cellar. Gauge was difficult to read at ground level but appeared to indicate little or no pressure if accurate. No obvious major issues were observed at this time.
2743485 Primary Facility 2018-06-19 Routine/Complete Inspection No Violations Noted The DEP inspected the EQT – Phoenix Site C Well Pad located in Duncan Twp, Tioga Co. on 6/20/2018 @1100 hours. The weather was 78 degrees, 45% humidity and winds N 12mph. The receiving waters are listed as Sand Run (CWF) wild trout stream. The well site is in production. No activity was observed onsite during this inspection.

The site consists of the access road, freshwater impoundment, and well pad on a total of 22.4 acres. The impoundment is drawn down and almost empty. There are 11 wells currently in production, and one plugged well. The area surrounding the well pad appeared stabilized. A plastic bag containing old foam berms has blown onto the south slope of the pad and should be removed.

No new violations were noted during this inspection.

2801218 Primary Facility 2018-11-07 Routine/Complete Inspection No Violations Noted The DEP conducted a Routine inspection of the EQT - Phoenix Pad C located in Duncan Twp, Tioga Co. on 11/6/2018 @1115 hours. The weather was 52 degrees, rain, winds NNE13mph. No company personnel was present onsite during this inspection.

The site consists of the access road, freshwater impoundment, and well pad on a total of 22.4 acres. The impoundment is drawn down and almost empty. There are 11 wells currently capable of production, and one plugged well. The area surrounding the well pad appeared stabilized.
