The last inspection conducted on behalf of the operator was on 8/13/2013: the only issue noted was that the cellars are holding water. There are seven drill permits issued for this site, the 1V and 2H wells have been drilled, the well tags installed and the permits are active. The status of the 1H, 3H, 4H, 5H and 6H well permits are listed as Operator Reported Not Drilled. The ESCGP is active. The site is well vegetated and stabilized. No puddles were observed on the pad surface and no issues were noted. No violations were cited during this inspection. |

Mr. Bernethy contact the WSQ @ 1150 hours on 1/26/2016 to notify the Department that SWEPI LP removed the temporary containment liner used during well completion activities on the 23 H well (API#117-21792) at the Stanley 1106 pad. As part of SWEPI LP’s standard procedures, soil conductivity readings were collected from the pad following the liner removal. Soil conductivity measurement points exhibited readings above background in the area of the temporary liner; approximately 6-8 mS. SWEPI LP was not notified during completion activities of any spills by the completions contractor, US Well Services. |

The weather was mostly cloudy 75 degrees with winds S 7mph. SWEPI Env. Coordinator, Pat Bernethy stated the production tank that was installed for the 23H well appeared to have a faulty gasket around the heater. This had allowed production fluid to seep into the permanent causing elevated conductivity in the accumulated fluid. The gasket was repaired and the fluid was removed from the containment. According to Mr. Bernethy ~1-2 weeks later a small area of damp soil appeared to be coming from underneath the permanent containment. The gravel inside the containment was removed and the liner was inspected for holes. Approximately 8 puncture holes and 1 large gash were observed along a 5 foot length of containment. The weather is 29° and cloudy northwest winds 13 mph. Three wells wereobserved drilled on the pad. Two wells appear to be in production. A large puddle near the well heads and some production equipment has a field measured conductivity of 150 microsiemens/cm. A new ESCGP-2 has been issued for the site to facilitate a pad build-out complete with installation of PCSM features. Cut and fill slopes appear stabilized by >70% perennial vegetation. The weather was partly cloudy and 41 degrees. The act 9 sign was observed at the site entrance. The E&S Plan was in the mailbox by the entrance. The inspection log was in the mailbox by the production tank and company personnel last inspected the site on 3/16/2019, noting no issues.
There is currently 2 producing wells onsite. The 2H is a Marcellus well and the 23H is a Utica well. The site is very well vegetated and stabilized. The reclaimed pony pad to the north was hardly noticeable. No erosion was observed onsite. Containment structures were intact and no leaks or spills were observed. PCSM BMPs were installed per the site plan. No violations or issues noted. I departed the site at 1215 hours. |
