2467901 Primary Facility 2016-03-23 Routine/Complete Inspection No Violations Noted Routine inspection. Site is permanently stabilized. A two-year extension on the restoration requirements was obtained by SWEPI on 6/18/15. TLT
2590324 Primary Facility 2017-05-04 Routine/Partial Inspection No Violations Noted EC w an Oakton PCSTestr 35: trailer containment--0.18 mS/cm, pad--0.30 mS/cm. I spoke w SWEPI Well Tender, Al Jaggie, on site.
2829365 Primary Facility 2019-01-18 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Shelman 291 Well Pad in Charleston Twp, Tioga County on 1/18/2019 at 1155 hours. The weather was 32 degrees, cloudy and 1/4 inch of snow covered the pad.

There are currently 9 wells that appear to be in production onsite (1H, 2H, 3H, 4H, 5H, 5HS, 6H, 7H, & 8H). There are three highland production tanks onsite. Fluids on the pad surface were frozen solid today. The steep banks looked well vegetated and stabilized. The rock-lined channels appeared to be constructed well. The pony pad to the north is currently empty. No erosional issues were observed today.

No violations noted today, I departed the site at 1135.
