Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2068697 Primary Facility 2012-05-17 Drilling/Alteration No Violations Noted
2149841 Primary Facility 2013-03-15 Routine/Complete Inspection No Violations Noted 1 gas well; large pad
2301743 Primary Facility 2014-08-27 Routine/Complete Inspection No Violations Noted
2644514 Primary Facility 2017-10-06 Routine/Complete Inspection No Violations Noted Routine pre-plug inspection of a gas well. The well was shut-in w/ master valve on b-section. Cellar below b-section filled w/gravel with four annular vents observable at surface. No obvious major issues were observed at surface.
2651924 Primary Facility 2017-10-25 Routine/Complete Inspection No Violations Noted Routine inspection of a well pre-plugging. Well is shut in with master valve on B section and cap. The cellar was full of gravel with standing water on top. No bubbling was observed in the water. There were four vent pipes to surface: two 2" inch vent pipes, a 1 inch vent pipe and one 1 1/2" inch vent pipe. I could not see where the vent pipes were coming from or if they were open in the cellar because the cellar was full of gravel. The operator intends to begin plugging tomorrow according to the notice of intent to plug submitted. No obvious major issues were observed.
2656265 Primary Facility 2017-11-01 Plugging(Includes Plugged/Mined Through) No Violations Noted Rig on location for P&A. The crew has just rigged up. The is dead. I spoke with the companyman and discussed tentative procedure. They'll drill out bridge plug at 3000', run in to 5600' to set a CIBP below the tully, cmt and recover production csg above TOC and plug remainder of well. No issues observed.
2658083 Primary Facility 2017-11-07 Plugging(Includes Plugged/Mined Through) No Violations Noted Rig on location for P&A which has just been completed. I spoke with the Company man and discussed operations. No issues reported. I walked over to well to inspect; 5 1/2" was recovered from 2810', I looked down in the 9 5/8"'casing and checked w/ Altair 5x gas meter. No issues observed, no gas was detected with an Altair 5x gas meter. I discussed depth of plugs with the Companyman. CIBP set at 5600, capped with cement back to 5400', gelled uphole , plugged back with cement from 4100' to 4376', cut and pulled casing at 2810 and plugged back from stub to 2365', spotted cement across surface shoe ( inside 9 5/8"). The well will be cut, capped, and buried below surface.
2662653 Primary Facility 2017-11-20 Administrative/File Review No Violations Noted November 20, 2017: Certificate of Well Plugging for the Youst 405 1H (117-21428) has been received, reviewed, signed, and sent to file.
2679970 Primary Facility 2018-01-09 Plugging(Includes Plugged/Mined Through) No Violations Noted Routine inspection of a recently plugged well. The well is cut, capped, and buried below ground level. No methane was detected at ground level at the well's surface location. No obvious issues were observed.
2786910 Primary Facility 2018-10-10 Plugging(Includes Plugged/Mined Through) No Violations Noted Routine inspection of an unconventional well plugged last year. The plugged well has been cut and buried below ground level. There were no open holes, discharges, venting, or other obvious issues observed at the surface location of the plugged well.
