Crews were in process of breaking down a drill rig in order to make way for a larger rig. All equipement and chemicals on site were within containment. 2535740 Primary Facility 2016-11-09 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess conditions during well completion activities.

A petroleum sheen was observed off of containment along the western side of the well site. The sheen extended from near the job operations trailer toward the southwest corner of the well pad. Most of the sheen area was silvery, with a brightly colored sheen closest to the job trailer. Two site personnel were spot cleaning with petroleum absorbent pads within the spill area upon my arrival. I informed Jonathan Sites of Southwestern, the on-site representative, of these findings, and recommended that additional clean up measures be implemented. I also contacted Afton Sterling, Southwestern Regulatory Manager, and informed her of my findings. She indicated that a crew would be dispatched to vacuum the puddles in the spill area and scrape the pad surface to remove affected material.

Containment was in place over the eastern 3/4 of the well pad. A workover rig was on site, and tank batteries are in place along the permiter of the containment. Electrical condcutivity of puddles on the well pad was screened using a Oakton PC Testr 35. One puddle in the northwest quadrant of the well pad was screened at 6.49 mS/cm; puddles throughout the rest of the pad not covered by containment (the western 1/4) ranged from 2 - 2.75 mS/cm. No puddles were observed to have a sheen or odor. Samples were collected for laboratory analysis (SAC 942) from the puddle with conductivity of 6.49 mS/cm and a puddle with conductivity of 2.04 mS/cm.

The elevated conductivity of the puddles could be indicative of a release of frac fluids, flowback, or produced water. Laboratory analysis of the samples collected will provide additional information regarding the chemical constituents of the sampled fluid, which will aid in

I entered the site at 10:00 with Derek Collins and Jamie Soto (DEP) and talked to the Company Man, Mr. Hale. At the time of the inspection, personnel onsite were rigging-up the Falcon 25 on the Lepley 4H (117-21832) well which was SPUD previously. Containment liner was installed under rig mats and the berm was being installed. Chemical storage was already marked and in containment. Two wells onsite were previously drilled and were protected by steel structures. The Lepley 3H (117-21800) was being produced. The site was stabilized and no E&S issues were noted. No Violations Noted.

Most of the pad surface is covered with containment. All equipment, fuel, additives, and materials are within secondary containment.

The electrical conductivity of puddles within and outside of the containment was screened and ranged from 170 to 495 uS/cm.

It was noted that site traffic is depositing mud on the public roadway (Milk Plant Road). It is recommended that the public roadway be swept clean to ensure safe travel conditions for the general public and site personnel.

2694273 Primary Facility 2018-02-12 Routine/Complete Inspection No Violations Noted Weather was 33° and sunny at time of inspection. The well tag attached to the well potentially has an incorrect API # listed on it. It reads 37-117-21728 and efacts has the 6H listed as 37-117-71722. Well has a digital gauge attached to the swab valve, the 5 1/2" casing and also to the 5 1/2 x 9 5/8 annulus event. The digital gauge attached to the swab valve read 428 psi, the 5 1/2 casing read 1066 and the annulus gauge read 261 psi at time of inspection. No other annulus vents were observed at time of inspection. No violations were noted at time of inspection.
2775135 Primary Facility 2018-09-11 Routine/Complete Inspection No Violations Noted Southwestern Energy (SWN) submitted a well site restoration report to the Department for the SWN TI-04-Lepley Pad. The report was received on 8/28/18.

I arrived at the access road entrance and was approached by a pipeline worker, Dirk. He asked if i was inspecting the pipeline construction then left.

Due to recent rain there was some ponding water on the pad. The water in one of these puddles had a conductivity of 156.6 µS/cm.

There are 4 properly tagged wells staged for production (Lepley 1H, 3H, 4H, & 6H). There are 4 GPUs onsite and in secondary containment. Water in GPU secondary containment had a conductivity of 160.2 µS/cm. There are 2 produced water tanks onsite and in secondary containment. Water in this secondary containment had a conductivity of 801 µS/cm. The smaller produced water tank appeared to be disconnected from all other equipment. The larger, connected tank appeared to have some rust colored staining near the top and rusting/holes near the bottom (Photos # 6 & 7).
