2404126 Primary Facility 2015-08-25 Administrative/File Review No Violations Noted On March 26, 2015, while conducting a site characterization following the stimulation of the 22H and 24H wells at the Cruttenden 846 well pad, field soil conductivity readings above 1.5 milliSiemens per centimeter were detected. Shell notified the Department of its findings that same day and elected to remediate the area affected area. From 3/2715 through 3/30/15 approximately 282.19 tons of gravel were excavated and disposed of at the Waste Management Landfill. On 8/25/15 the Remedial Action Completion Report was submitted to the DEP and documents the post remediation sample locations, sample results and disposal receipts for the excavated material. The report states that the analytical results of the post-excavation soil samples collected on 3/20/15 exhibited concentrations less that the most stringent Medium Specific Concentrations (MSC). The document will be handled in accordance with the Alternative Remediation provision of the Department policy document 800-5000-001 Addressing Spills and Releases at Oil and Gas Well Sites or Access Roads

2509140 Primary Facility 2016-08-08 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection inspected the SWEPI LP – Cruttenden 846 well pad in Middlebury Twp., Tioga Co. on 8/8/2016 @1300 hours. The weather was 80° and sunny. An adequate site ID sign was located at the entrance to the access road. An E&S inspection was last conducted by company personnel on 8/2/2016 with “no change” noted in the comments section. A small erosion channel was visible along the south side of the access road. Silt sock was visible along the access road to the tank pad.

Secondary containment around the produced fluid tank contained a small amount of water with a field tested conductivity of ~700µS/cm. A hose connected the produced fluid tank to a battery of temporary tanks, located on containment, on the NE corner of the pad. Two large puddles were observed between the production equipment and the battery of temporary tanks. Puddle 1 had a field conductivity of 4.17 mS/cm. Puddle 2 had a field conductivity of 3.2 mS/cm. 2513050 Primary Facility 2016-08-19 Follow-up Inspection No Violations Noted The Department of Environmental Protection conducted a follow-up inspection at the SWEPI LP – Cruttenden 846 well pad in Middlebury Twp., Tioga Co. on 8/8/2016 @1230 hours. The weather was 78° and sunny. An adequate site ID sign was located at the entrance to the access road. An E&S inspection was last conducted by company personnel on 8/172016 with “some small erosion channels on access road” noted in the comments section. This issue was also noted in the previous inspection report dated 8/8/2016.

As of today’s inspection approximately 5 puddles were observed on the pad surface in approximately the same locations as noted in the previous inspection. The field conductivities ranged from ~1.9 mS/cm – 2.4 mS/cm. Conductivity was tested with a Eutech Instruments Oakton Multi-Parameter PCSTestr 35. The hose connecting the permanent produced fluid tank to the temporary tank battery was laying in the tank containment. There are currently 2 actively producing wells onsite with one production tank. The pony pad to the West is being used to store some piping and some farm equipment. A puddle on the pad surface (after ice was broken) had a field measured conductivity of 490us/cm. No erosional issues observed. The cut slope to the East is well vegetated and appears to be stabilized.

No violations noted today, I departed the site at 1450.
