The weather was 50 degrees and sunny. The site is currently under construction. An adequate site ID sign was observed at the entrance to the access road. The well permit for API# 37-117-21813 was located in the mailbox along with the E&S plan. The receiving stream is Broughton Hollow UNT to Cowanesque River, UNT to Broughton Hollow, and UNT to Jemison Creek. The total disturbed area is listed as 28.1 acres. The last site inspection report in the mailbox was dated 11/12/2015 and was preformed after a stormwater event. The comments section stated “There’s a few CFS that has some silt trapped, we will clean out once it dries up.”
2433556 Primary Facility 2015-12-15 Preoperation Inspection No Violations Noted The Department inspected the Travis Peak Resources LLC site, the Painter Well Pad, ESX29-117-15-0023, located in Westfield Twp., Tioga Co. on 12/15/2015 @ 1030 hours. The weather was 45 degrees and overcast. An adequate site ID sign was observed at the entrance to the access road. The well permit for API# 37-117-21813 was located in the mailbox along with the E&S plan. The receiving stream is Broughton Hollow UNT to Cowanesque River, UNT to Broughton Hollow, and UNT to Jemison Creek. The total disturbed area is listed as 28.1 acres. Construction of the site is complete and the access road was receiving it's final layer of 2A stone during the inspection. The conductor pipe and well cellar was delivered to the pad during the inspection.

2471575 Primary Facility 2016-04-05 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the Travis Peak Resources LLC – Painter Well Pad (ESX29-117-15-0023) located in Westfield Twp., Tioga Co. on 4/5/2016 @1030 hours. The weather was 25° sunny with 53% humidity, winds WNW 8 mph. A valid site ID sign was observed at the entrance to the access road. The onsite inspection log was located in the mailbox. The site was last inspected by Mr. Taylor on 4/2/2016 with no comments listed. The receiving waters is olisted as Broughton Hollow (UNT to Cowanesque River), UNT to Broughton Hollow, UNTs to Jemison Creek/WWF. Approximately 2-5 inches of snow was observed on the ground in some areas. Mike Taylor, Well Versed Oilfield and Therman Hudson, Rig Manager H&P 500 were present during the inspection. No violations are noted at this time. I departed the site @ 1230 hours.
2574162 Primary Facility 2017-03-22 Routine/Complete Inspection No Violations Noted The DEP inspected the Travis Peak Resources Painter well site in Westfield Twp. Tioga Co. on 3/22/2017. The weather is 24 degrees and sunny. Express is in the process of flowing back the 1H well. The pad surface is a mix of frozen ground and mud. Primary containment surrounding the well head appears to be folded over and creased in some areas. E&S controls appear functional. No violations noted at this time. 2700784 Primary Facility 2018-02-27 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection conducted a routine inspection of the Eclipse Resources – Pa, LP Painter Well Pad located in Westfield Twp., Tioga Co. The weather was 50 degrees and partly sunny. Present for my inspection were Bruce Carpenter, Manager EHS and Regulatory; Alex Duranko Regulatory Specialist; Zac Subra, EHS.

Eclipse Resources recently acquired the Painter Well Pad from Travis Peak, LLC. Access to the site has been limited by the addition of a guard shack along with security personnel on the access road. The pad surface has been regraded and had extra top rock added to remediate the muddy conditions on the surface. At the time of my inspection, Eclipse was preparing to set the conduction on the 2H well. The 1H well was drilled by Travis Peak and is currently in production. Production fluids are stored in two upright tanks in containment. No evidence of staining, leaks or corrosion was noted on the tanks. Some fluid was observed in secondary containment. 2722057 Primary Facility 2018-04-23 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection conducted a routine inspection of the Eclipse Resources LLC –Painter Well Site located in Westfield Twp., Tioga Co. on 4/23/2018 @ 1300 hours. The weather was 65 degrees and sunny. Zachary Subra, Eclipse Resources EHS Field Rep was present onsite for the inspection.

Onsite personnel were currently in the process of “rigging down” in preparation for moving the drilling rig to another location. A large crane and multiple escort and transport vehicles were staged onsite. Components of the rig were being pressure washed on the main containment before being hauled offsite. One tank of brine was observed on the main containment leftover from drilling. The mud tanks still contained drilling mud. Those tanks were scheduled to be emptied as part of the removal process. The mud would be sent to another drilling location. Puddles on the ground surface were screened for conductivity and found to be within normal range.

2726663 Primary Facility 2018-05-03 Routine/Complete Inspection No Violations Noted The DEP conducted an ICDNT inspection at the Eclipse Resources – Painter Well Pad on 5/3/2018 @1030 hours in response to spill #333923, reported on 5/1/2018. Zachary Subra of Eclipse Resources called to report a subcontractor had used 40 bbl of brine water, leftover from drilling , to wash down the rig mats on containment. Mr. Subra stated, due to holes in the containment an unknown quantity of the brine was released to the pad surface. Screening and excavation of impacted pad surface material commenced on Tuesday, 5/1/2018. Initial field readings of the impacted material were 80-100 mS/cm.

Upon transfer of the Painter well pad from Travis Peak, Eclipse had brought in material and regraded the pad surface to establish positive drainage away from the cellars. The NE quadrant of the pad surface was observed excavated down between 1’-2’ in some areas. Mr. Subra stated the brine infiltrated the top material, hit the compacted original surface layer, and migrated towards the well cellars. Material was stockpiled

2734033 Primary Facility 2018-05-24 Follow-up Inspection No Violations Noted The DEP conducted a follow-up inspection at the Eclipse Resources Painter Well Pad located in Westfield Township, Tioga County on May 24, 2018 @1030 hours. The weather was 71 degrees and sunny. Winds WNW 6mph.

Zach Subra and Alex Duranko, Eclipse Resources EHS were onsite during the inspection. Penn E&R consultants were onsite collecting confirmatory samples from two areas on the pad surface, and the forebay of the sediment trap in relation to Spill #333932 reported on 5/1/2018.

The excavated areas are still open. All rainwater coming into contact with excavated areas of the pad is pumped from the sump into rolloff containers staged on containment. Any fluid accumulating in the four bay of the sediment trap is also being pumped into separate rolloff containers staged on containment. The recovered fluid is being sent to Hydro recovery for disposal . The excavated pad material is in tarped waste containers, staged on containment, awaiting disposal approval.

2759034 Primary Facility 2018-07-31 Follow-up Inspection No Violations Noted The DEP conducted a routine inspection of the Eclipse Resources – Painter Well Pad located in Westfield Twp., Tioga Co. on 7/31/2018 @1215 hours. The weather was 78 degrees and cloudy. Korey Starkey, DEP Env. Trainee and Zac Subra,Eclipse Resources EHS Field Rep were present for the inspection.

Halliburton was onsite performing a frack on the 2H well (API#117-21907). Mr. Subra stated completion activities began on 7/24/2018 and he believes Haliburton will be on location for another 10 days. Due to heavy precipitation in the past week, the pad surface was very muddy with standing water in some areas. All puddles screened for conductivity were <1000µS/cm.

No violations are noted at this time. I departed the site at 1345 hours.

2790018 Primary Facility 2018-10-16 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of Eclipse Resource's Painter Well Pad in Westfield Twp, Tioga County on 10/16/2018 at 1230 hours. The weather was partly cloudy and 49 degrees.

The pad is currently under flow back operations. Onsite company personal was currently monitoring the flowback. Two production tanks and frac tanks were located onsite within secondary containment. All fluids field measured were within normal conductivity ranges.

No violations noted at this time. We departed the site at 1305 hours.

2841591 Primary Facility 2019-02-19 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) conducted a routine inspection of the Eclipse Resources - Painter Well Pad located in Westfield Twp, Tioga Co. on 2/19/2019 @1030 hours the weather was 17 degrees, winds NW @10 mph, humidity 43%. There were two wells in production on the pad. No activity was observed. Zach Subra, Eclipse Resources was present onsite for the inspection.

No violations are noted at this time. I departed the site @ 1130 hours.
