2596468 | Primary Facility | 2017-05-23 | Routine/Complete Inspection | No Violations Noted | I arrived on-site @ 11:30 hrs. and met Brett McCoy, SWEPI company man. Ensign rig 192 on location drilling the 5H well; Mr. McCoy expected to be at TD sometime on Tuesday (currently @ 11460'). The foam berm had gravel added in drive-over areas, and in spots was compressing the berm. Mr. McCoy and I discussed some long-term solutions for this, including adding a hard berm such as a timber in these areas. As it was impossible to tell how high the berm still was, no violation will be issued for insufficient containment, but I recommend that this potential issue be taken into account for the move to the Lingle pad. I departed the site @ 12:20 hrs. |
2616548 | Primary Facility | 2017-07-18 | Incident- Response to Accident or Event | No Violations Noted | Spoke with Dean Albright (SWEPI) on site. He and Keystone Clearwater explained that an open valve on the small pad allowed fracturing liquid flow through the plastic pipe to the battery of 30 wheelie trailers on liner containment on the well pad last night. The liquid overflowed from the top of 1 of the wheelie trailers. They removed most of that liquid; the residual liquid on the liner gave reading of "Or" (>19.99 mS/cm) with an Oakton PCSTestr 35. The highest reading of liquid off containment at this site was 0.90 mS/cm. US Well Services plan to start fracturing operations on the 5 Marcellus wells tonight. |
2619542 | Primary Facility | 2017-07-21 | Incident- Response to Accident or Event | No Violations Noted | EC readings with PCSTestr 35 (mS/cm): near water line breach--0.31, small pad containment--0.16, pad--0.75, well pad containment where fracturing liquid spilled on July 17, 2017 ( inspection record 2616184)--15.5. |
2629802 | Primary Facility | 2017-08-23 | Routine/Complete Inspection | No Violations Noted | I met Matt Jentsh and Rafael Diaz when I arrived on-site. Frac had ended earlier in the day, and most fracing equipment had already been removed from the site. A sitewide liner was in use with 4" foam berms for containment. On the west side of the pad between the foam berm and the soil berm, I discovered numerous puddles with elevated conductance (see sketch for details). A large volume of water showing elevated conductance (12-18mS/cm) was present in containment at the foam berm immediately next to these puddles. I continued field screening puddles on my way down to the sediment trap, finding puddles of 2.2 and 2.8 mS/cm on my way; conductance readings in the sediment trap were much lower (.595 mS/cm). I then discovered a large puddle (20-25 gallons) on the pad surface off the north side of the containment that showed elevated conductance (4.6-10.2 mS/cm). I informed Matt and Rafael of what I had found, and Matt immediately got a vac truck driver to go to the areas I located on the west side of the p |
2638466 | Primary Facility | 2017-09-21 | Administrative/File Review | No Violations Noted | I received a final analysis from the Departmen's Bureau of Laboratories on 9/21/2017 for sample #0939053, which I took at the Cotton Hanlon pad on 8/23/2017 from a puddle exhibiting elevated conductance. Elevated barium (781.8 mg/l), strontium (533.7 mg/l), and chlorides (11474 mg/l) indicate that a pollutional substance was released to the pad surface. Violations assigned to API # 117-21866. |
