2629819 | Primary Facility | 2017-08-23 | Routine/Complete Inspection | No Violations Noted | I met Matt Jentsh and Rafael Diaz when I arrived on-site. Frac had ended earlier in the day, and most fracing equipment had already been removed from the site. A sitewide liner was in use with 4" foam berms for containment. On the west side of the pad between the foam berm and the soil berm, I discovered numerous puddles with elevated conductance (see sketch for details). A large volume of water showing elevated conductance (12-18mS/cm) was present in containment at the foam berm immediately next to these puddles. I continued field screening puddles on my way down to the sediment trap, finding puddles of 2.2 and 2.8 mS/cm on my way; conductance readings in the sediment trap were much lower (.595 mS/cm). I then discovered a large puddle (20-25 gallons) on the pad surface off the north side of the containment that showed elevated conductance (4.6-10.2 mS/cm). I informed Matt and Rafael of what I had found, and Matt immediately got a vac truck driver to go to the areas I located on the west side of the p |
2638551 | Primary Facility | 2017-09-21 | Administrative/File Review | No Violations Noted | I received a final analysis from the Departmen's Bureau of Laboratories on 9/21/2017 for sample #0939053, which I took at the Cotton Hanlon pad on 8/23/2017 from a puddle exhibiting elevated conductance. Elevated barium (781.8 mg/l), strontium (533.7 mg/l), and chlorides (11474 mg/l) indicate that a pollutional substance was released to the pad surface. Violations assigned to API # 117-21866. |
2658389 | Primary Facility | 2017-11-08 | Routine/Partial Inspection | No Violations Noted | I spoke with Kevin Morgan (SWEPI , Flowback OSR) on site. He said the row of trailers (photo) contains production liquid form the 5 Marcellus wells on site.
Air temp: 32F. Some ice on top of puddles today. Electrical conductivity (mS/cm) readings taken with an Oakton PCSTestr 35. JLP |

2677784 | Primary Facility | 2018-01-03 | Routine/Partial Inspection | No Violations Noted | No electrical conductivity readings today because I did not see any standing liquid on site. JLP |
2696287 | Primary Facility | 2018-02-21 | Routine/Complete Inspection | No Violations Noted | At time of inspection all the wells on the pad were being flowed back
Unable to check annulus vents for combustible gas at time of inspection due to flowback being done |
2706158 | Primary Facility | 2018-03-15 | Routine/Partial Inspection | No Violations Noted | Site snow covered. I did not see any standing liquid on site. SWEPI inspection log in mailbox near pRoduction tanks lists last inspection on March 12, 2018. Air temp 29F. JLP |
2788364 | Primary Facility | 2018-10-03 | Site Restoration | No Violations Noted | The DEP conducted a RESTORATION inspection of SWEPI's Cotton Hanlon 595 Well Pad in Sullivan Twp, Tioga County on 10/3/2018 at 1400 hours. The weather was 64 degrees, 71% humidity and cloudy.
The inspection was conducted in response to a Notice of Termination received by the DEP. The pad appears very well vegetated and the pony pad to the North is completely restored. All PCSM BMPs are in place and appear to be functioning properly. No temporary E&S BMPs were observed. There are currently 5 producing wells onsite with two highland production tanks. This Notice of Termination is recommended for approval. No violations noted at this time. I departed the site at 1435. |
