2654853 Primary Facility 2017-10-27 Routine/Complete Inspection No Violations Noted I met Jonathan Woodburn on-site. SWN rig # 45 running production casing in the 5H well. Cuttings processing was idle. Containments appeared to be in good condition. The site was somewhat muddy, and Woody told me he had a vac truck scheduled to clean things up.
2725609 Primary Facility 2018-05-01 Routine/Complete Inspection No Violations Noted I met Joe Forker, SWN company man when I arrived. A workover rig was on the site, and had been drilling out plugs on the 1H well; when I arrived, the rig was tripping out of the well. Joe informed me that they would be deploying fishing tools into the well to recover some live perf guns that were lost in the 8th stage. Joe walked the site with me. A 50-70 gallon puddle on the pad in a high traffic area had slightly elevated conductivity 2.36 mS/cm); all other puddles on the pad were significantly lower (.3- .9 mS/cm). I recommended to Joe that he have it vacced, and while I was still on the site Joe directed a vac truck driver to move to the puddle and clean it up. The containment and berms seemed to be in good condition.
2833298 Primary Facility 2019-01-28 Routine/Complete Inspection No Violations Noted Weather was 21° and sunny at time of inspection. There were no Southwestern representatives on location at time of inspection. A containment crew was on location laying containment for the drilling rig to move on. Well was a spud conductor. Well was covered by containments at time of inspection. No violations were noted at time of inspection.
2841885 Primary Facility 2019-02-11 Routine/Complete Inspection No Violations Noted Weather was 25° and snowy at time of inspection. Met with Southwestern representative Dan Hessdorfer on location. Well was a spud conductor only. Well was covered with rig equipment at time of inspection. No violations noted at time of inspection.
2842710 Primary Facility 2019-02-21 Routine/Complete Inspection No Violations Noted Weather - 40 degrees and sunny

Met with Southwestern representative Andrew Griffith. Well was a SPUD conductor. Well was cover with a steel plate and rig equipment at time of inspection. This well will be completed after the finish on the 4H. No violations noted at time of inspection.

2849582 Primary Facility 2019-03-06 Routine/Complete Inspection No Violations Noted Weather is 16 degrees and cloudy. Met with Southwestern representative Andrew Griffith and Dan Hessdorfer. Well recently had is surface and intermediate casing set and cemented. Reviewed the cement job logs and no discrepancies were noted at time of inspection. It was discovered that The 6H was on a collision with the 5H. Gyro Data was on location setting a mechanical whipstock plug from 2098' to 2128'. They were still waiting for permit approval before proceeding with drilling operations. No violation were noted at time of inspection.
2852995 Primary Facility 2019-03-13 Routine/Complete Inspection No Violations Noted Weather was 30 degrees and sunny. Met with Southwestern representative Dan Hessdorfer on location. Rig recently had to stop drilling due to discovering that they were on a collision course with another lateral on location. Southwestern decided to set a whipstock plug and sidetrack their wellbore to correct their course. The top of the whipstock is at 2098' and the bottom is at 2128'. They successfully sidetracked their wellbore and were currently drilling ahead in the curve. The estimated TD is 13615'. No violations at time of inspection.
