2800633 Primary Facility 2018-11-08 Routine/Complete Inspection No Violations Noted An inspection of the Cupper 10H well was conducted November 8, 2018. I arrived on location at approximately 11:00 am. A directional rig is on location drilling an unconventional well. The rig was flaring the well at a flare-stack on the edge of the pad at the time of my arrival. I met with the Companyman to discuss operations on the pad. Surface/intermediate casings have been set and the crew has been drilling the production section of the wellbore. They reportedly encountered pressure last night while drilling on air in the Centerfield Limestone at a TVD of ~6842' (7473 MD). They were able close annular, divert well through choke, flare well, and shut-in well where 1923 PSI was recently observed. At this time they will attempt to bled pressure from casing furthermore to pump water and are prepared to pump heavier OBM at this time. 9 5/8" casing shoe is at 1995'; cement pump chart and FIT were reviewed, as well as BOP, annular, and choke tests. 2806681 Primary Facility 2018-11-19 Routine/Complete Inspection No Violations Noted Weather was 36 and snowing at time of inspection. Met with Johnathan Woodburn on location. Rig was in the process of moving to the 5H. Rig completed cementing the production casing on this well at 9pm on 11/18/18. Reviewed the cement job log from the production casing and no discrepancies were noted at time of inspection. Well was closed in with a night cap. No violations were noted at time of inspection.
2809694 Primary Facility 2018-11-28 Routine/Complete Inspection No Violations Noted Weather was 36° and snowing at time of inspection. Met with Andrew Griffith on location. Rig was in the process of drilling the 5H at time of inspection. Well was closed in with a night cap. I was able to observe the cellar and no bubbling was noted. No violations were noted at time of inspection.
2816065 Primary Facility 2018-12-17 Routine/Complete Inspection No Violations Noted Weather was 37° and cloudy at time of inspection. There were no Southwestern representatives on location at time of inspection. Containment crews were on location removing rig matting. Well was recently drilled and cemented. Well was shut in with a night cap. Surface and intermediate casing annuli were visibly cemented to surface at time of inspection. No violations were noted at time of inspection.
2845871 Primary Facility 2019-02-27 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected Inspection conducted in response to a spill notification received yesterday 2/26/19 about a release of an estimated 10 gallons of produced water that had occurred at 1730 the same day. Mike Deming of SWN met me on location upon my arrival. He explained that an 18-wheeler off-loading produced water from a battery of frac tanks had inadvertently pumped produced water out of the truck into the tank instead of pumping it out of the tank and into the truck. This activated a pressure relief valve on the truck which led to the release of the produced water. Mr. Deming said that a vacuum truck on site immediately sucked up the fluid and gravel from the pad surface was scraped with a skid steer and placed into a container for disposal. At the time of this inspection, the site was snow-covered and the impacted gravel material had already been removed.
