2807541 Primary Facility 2018-11-28 Complaint Inspection No Violations Noted Complaint information states that complainants are concerned with a strong odor of crude oil. During the well inspection, the crude oil odor was present. The location of the spill could not be determined due to the deep snow, but the wellhead or any associated production lines or fittings were not leaking. WQS Johnson is currently working with operator/landowner to clean up the spill, which will eliminate the odor of crude oil.

Well is registered to James T Larson in Department database. Current landowner is using well for home use gas under the assumption that the well was part of the property sale. Will follow up with ownership verification at the county courthouse.

11/26/18 Before the complaints had been logged, AQS Melanie Lewis and my self visited one of the complainants at their residence to perform a site investigation. This investigation was conducted at approximately 14:00. The odor was not evident at the time of this inspection. There were, however, small amounts of oil seen to be traveling down a near by ditch-line. The oil was traced to a 140 bbl steel tank that was positioned near well # 123-32108. It was not clear where on the tank the oil was leaking from. Straw had been placed over much of the impacted area. The oil appeared to have spread out over a relatively large area, just below gradient of the tank. The oil then appeared to have been concentrated by a french drain, some ways down g

11/30/18 I arrived at the Johnson residence at approximately 15:30 and met with Justin Johnson. Mr. Johnson confirmed when the spill had occurred, and that the volume was believed to have been small, but unknown exactly. He also pointed out that the leak had occurred from the heat-treating pipe opening, at the base of the tank. No oil was visibly leaking from the pipe at that time, but I suggested that the tank be tipped on its side to ensure that no further oil leaked out. I recommended that the Johnson's remain proactive in the clean-up process, by continuing to change pads, possibly applying some peat moss in some areas, and continuing to skim oil that was collecting in the sumps.
