2851977 Primary Facility 2019-03-08 Routine/Complete Inspection No Violations Noted The main access road entrance for this location is at the end of Six Mile Rd., in Mead Township, Warren County, PA. The location can also be accessed, on foot, from the terminus of Forest Route 365.

This is a combination oil/gas producing well, fitted with a gasoline powered pump-jack. Both, main valves from the production, and casing strings appear to be closed. There is no evidence of any major spills or releases around the well head. There are no clear identifiers on the well head or pump-jack. The API ID, should clearly be tagged to the well as per current regulations. The ground is currently snow covered. This location does not appear to have been accessed for some time. A good deal of vegetation can be seen poking through the snow, both on the accessing road and around the location. E & S BMP's appear to have been adequately implemented when the location was initially constructed.
