2234109 Primary Facility 2014-01-06 Routine/Complete Inspection No Violations Noted
2770591 Primary Facility 2018-09-04 Routine/Partial Inspection No Violations Noted Inspection is in response to an email received from USFS regarding a gas leak at a compressor station on ANF land. A complainant emailed the USFS with concerns of a gas leak that was detected using an optical gas imaging camera. It was stated in the email that the gas was noted to be coming from a regulator on one of the lines from the compressor. Gas was detected within approximately 5' of the compressor at a maximum concentration of 5% of the lower explosive limit. At further distances from the compressor, an odor of gas was present, but it was not at high enough concentrations to bump the gas meter.

It was also stated in the complainants email, that a strong odor of gas was noticed coming from the well near the compressor. The well was checked at all possible points of gas escape with an Altair 5x gas meter. Gas was detected at a concentration of 25% of the lower explosive limit in the annulus of the surface casing. A review of the well record indicates cement return to surface during the drilling phas

2770782 Primary Facility 2018-09-05 Complaint Inspection No Violations Noted Incident has been referred to DEP Northwest Regional Office for investigation. Inspection dated 09/04/2018, inspection ID# 2770591, will be used as initial response detailing the Departments findings. Investigation is currently ongoing. Enforcement action will be taken if necessary. - Invalid Complaint Id. - 3366675
2774620 Primary Facility 2018-09-13 Follow-up Inspection No Violations Noted Surface casing has been vented to atmosphere. Bubbling still present on surface casing annulus, methane level at 55% L.EL. as indicated on Altair 5x gas meter. Will continue to monitor periodically for a week to rule out possibility of residual gas escaping cement sheath. If gas on surface casing annulus persists, further mitigation efforts will be necessary.
2777930 Primary Facility 2018-09-21 Follow-up Inspection No Violations Noted Surface casing continues to vent. Gas bubbles on casing annulus are still present but seem to be at a lesser rate than the previous inspection. Operator submitted plan of correction under 78.86 on 09/19/2018. The surface casing is believed to be leaking at a coupling 20-40' downhole. Production pipe will be set on a packer below this point and the annulus will be vented to atmosphere. Weekly inspections will continue until defect is corrected.
2791322 Primary Facility 2018-10-19 Follow-up Inspection No Violations Noted Gas on surface casing annulus has diminished significantly. The well will not completely blow down to atmospheric pressure. The cause of the leak has been attributed to a leaking coupling near surface. I informed the operator that the vent valve could be closed until a service rig could be brought in to run a packer in the well. With the leak located inside the conductor pipe, there is no risk of gas migration outside of the well.
2806877 Primary Facility 2018-11-27 Routine/Complete Inspection No Violations Noted 4 1/2" production casing has been set on a packer below the surface casing string. Surface casing has been left open to atmosphere. There is no longer gas venting on the outside of the surface casing. Production casing and tubing are connected to production lines.
