2815290 Primary Facility 2018-12-12 Routine/Complete Inspection No Violations Noted This location can be accessed on Forest Route 155A in Pleasant Township, Warren County, Pa. The location has been constructed as a bump out, on the cut bank side of FR 155A.

The contractors that are on sight are currently performing some of the final phases of the fracking process. Frack pipe is currently being removed from the well bore. All fluid returns appear to have been directed to the cut bank side of the location, and re-directed, via trench, to a pit in front of the service rig. A back splash has been placed on the bank across from the initial discharging point from the well head. The areas under the rig floor, in the trench line, and in the frack pit, have been covered with plastic liner material. All waist material appears to have been managed effectively. Relevant E & S related BMP's seem to have been implemented for this location. The accessing road seems to be holding up well to the heavy truck traffic. The permit for this well has been posted on a tree, near the location.

2816581 Primary Facility 2018-12-18 Routine/Complete Inspection No Violations Noted Well recently stimulated. Leadline ditch has been dug to well and lines have been laid and connected to well head. Well head valves are closed at this time.
