Present at time of inspection was Jared Stemple and Jay Kendra (HG). The site is located in South Franklin Twp., Washington Co. The inspection was related to a request by the operator to meet on site to discuss a slid which has developed on the site along the access road and start of construction of the PCSM structures.
The operator plans to re stabilize the fill slope along the access road which recently failed. After the slide repair the operator plans to begin installation of the PCSM structures on the site and progress termination of the ESCGP2 permit. 5 wells have been drilled to completion on the site and are equipped for production. |

A file review was conducted by John Carson and David McDermott (DEP) on 11/8/2018 regarding a spill of drilling based mud to the graveled well pad. It was determined during the review that a Compliance Evaluation Inspection was required in order to close out the incident. The following was observed during the 11/8/2018 inspection:
