2764816 Primary Facility 2018-08-13 Routine/Complete Inspection No Violations Noted On 8/13/2018 at approximately 1200, I conducted an incident response inspection of the Burkett Well Site located in Jefferson Township, Washington County in response to failure of BMPs on site from a large rainfall event. I met with Nate Smiell (Range Resources) on site to discuss the damage from the rain event and the associated remediation plans by Range Resources.

According to Mr. Smiell, there was approximately 3.5 inches of rain over the previous weekend which had caused a large amount of headwater pressure to build up on the newly constructed retention pond to the southwest of the site. The wall of the pond had failed due to excessive water buildup which resulted in a release of the water to the southwest of the site and eventually to UNT to Cross Creek. Additionally, two sections of 24" CFS located down gradient of the retention pond outlet were compromised. Damage to the access road and to lined channels leading to the retention pond were also observed during the inspection. The operator planned to

2807241 Primary Facility 2018-11-27 Routine/Complete Inspection No Violations Noted On 11/27/2018 at approximately 0920, I conducted a follow-up inspection of the Burkett Well Site located in Jefferson Township, Washington County. A retention pond failure was initially observed on 8/13/2018 following an isolated storm which resulted in accelerated erosion and sedimentation of the down-gradient watercourse. Repairs were made to the retention pond as well as the compromised E&S controls on site following the observation of the failure and were completed by 8/20/2018. During the 11/27/2018 follow-up inspection, the following was observed:
  • The Act 9 sign located at the front entrance was in good condition.
  • All wells were clearly marked and appeared to be in good condition.
  • All GPUs appeared to be in good condition.
  • Production fluid/condensate tanks were within secondary containment and appeared to be in good condition.
  • Retention basin to the South of the well pad appeared to be functioning properly and in good condition.
  • E&S controls installed beneath the retention pond appear
