| 2551821 | Primary Facility | 2017-01-03 | Incident- Response to Accident or Event | No Violations Noted | On January 3, 2017, I inspected the Papa Bear well site accompanied by OGI Supervisor Scott Sabocheck, OGI Eric Peterson, and OGI John Rushin as well as a number of Rice Energy representatives in response to a fire and subsequent loss of brine and other fluids to the pad that occurred on 1/1/17. Observations from the inspection are below.
•At the stream crossing on the site access road substantial amounts of gravel and mud had been sprayed over the CFS and into the immediate area of the stream. The issue was communicated to site personnel at the time of inspection and immediate actions were taken to remediate. No violation at this time. •The fire and associated efforts to extinguish it resulted in currently unknown amounts of brine, oil, firefighting chemicals, and other pollutants reaching the surface of the pad via numerous holes burned in the liner. This constitutes a violation of 25 Pa. Code § 78.57(a) and 25 Pa. Code § 78.54 in that production fluids were spilled to the ground. •At the time of inspe |
