Moved onto this well on 3/8/16 and resumed drilling at 50 ft. KB. The 20” casing was set at 281 ft. on 3/8/16 and cemented to surface with 26 bbls returned. The 13 3/8” casing was set at 512 ft. on 3/10/16 and grouted to surface above the basket. A mine was found from 464 ft. to 476 ft. A basket was set at 439 ft. Currently drilling ahead at 1536 ft.

KOP was at 2500 ft. The 9 5/8” casing was set at 2339 ft. on 3/12/16 and cemented to surface with 15 bbls returned. TD was at 7183 ft. MD on 3/18/16.

2593001 Primary Facility 2017-05-12 Incident- Response to Accident or Event In Compliance with Policy On 5/4/17 an INCDT inspection was conducted of the Shotski well site in response to an operator reported incident CTS ID ???. The following was observed:

>>The initial incident report indicated that 12-15 gallons of production fluids had been lost to the site access road. Communication with site personnel indicated that the spill was near the production tanks and that it had been cleaned up immediately with all impacted materials had been removed. New gravel near the loading point at the production tank corroborates the on site report.

>>Four wells were observed to be in production at the time of inspection. Eight boxes were observed on site as well.

2723194 Primary Facility 2018-03-09 Complaint Inspection No Violations Noted On 3/9/18 a complaint inspection was conducted of the Shotski well pad and an associated location in relation to a complaint (CTS ID: 333603) in which excessive water leaving the site resulting in erosion damage was reported. The following was observed:

>>The complainant reported that from roughly six weeks prior to the inspection date during multiple occurrences heavy rain overwhelmed site controls resulting in severe flooding in area streams and springs resulting in substantial property damage. Inspection of the related area found substantial erosion damage related to streams flowing from the direction of the well site.

>>Inspection of the pad and perimeter controls did not find current or historical evidence of sedimentation or erosion damage leaving the site and site controls appeared to be fully intact and functional. Water flow patterns from the site could not be observed at the time of inspection due to insufficient precipitation. No violation.

2723249 Primary Facility 2018-04-17 Incident- Response to Accident or Event In Compliance with Schedule On 4/17/18 an incident inspection of the Shotski Well Site was conducted in response to an operator reported spill. The following was observed:

>>On 4/14/18 EQT's Corey Giles reported a 10 gallon produced water spill at the Shotski Pad. No further information was provided at the time.

>>Inspection of the pad found no evidence of the spill. Contact with Corey Giles indicated that the spill was related to driver error while servicing the production tanks on site and that the spill was localized to the area around the truck transfer point near the tanks. The spill was reported as having been cleaned up at the time of inspection but clean up could not be verified. No new gravel or excavation was discernible at the time of inspection.

>>A follow up inspection will be provided for this spill when cleanup documentation and results are made available.

2735128 Primary Facility 2018-05-17 Follow-up Inspection No Violations Noted On 5/17/18 a follow up inspection of the Shotski Well Site was conducted in response to CTS ID: 332745. The following was observed:

>>No evidence was found of channeled water or erosion related sediments leaving the site. All site controls were found to be intact and appeared fully functional. No violations.

2840400 Primary Facility 2019-02-12 Routine/Complete Inspection No Violations Noted On 2/6/19 a complaint inspection of the Shotski Well Pad was conducted in response to a complainant reported incident. The following was observed:

>>The complainant reported ongoing flooding associated with the Shotski site.

>>Inspection of the complainant's location found heavy flooding causing some property damage. Flooding was endemic to the general area at the time of inspection related to heavy rain events.

>>Site inspection found that site controls all appear to be functioning with minimal overflow from the sediment pond on site. No channeled or otherwise inappropriate flow of water or sediment from the site was found. Water was visibly accumulating off site from surrounding pastureland into the channel and subsequent flood plain.

>>Immediate initial response completed to ensure public safety by inspector formerly responsible for area.
