•Along the access road, where it crosses the last saddle before the pad, E&S controls have been heavily reinforced partially in response to department recommendations. •Inspection of the area below the reinforced controls found multiple streams and waterways not shown in the site plans or in the original streams and wetlands report for the site. Due to this the site is not in compliance with §102.4(b)(4)(ii),(iv), §102.8(b)(2),(3),(5), and (8), §102.4(b)(5)(v) and §102.5(m)(2)(ii). •Many of the stream channels in the area adjacent to the site appear to be undergoing increased erosion and sedimentation.
