>>The operator reported on 2/5/18 that a hammer joint on a GPU failed resulting in the release of roughly 10 gallons of condensate to the pad surface. This constitutes a violation of 25 Pa. Code § 78.57(a) in that the operator failed to prevent the release of well fluids to the ground. The violation was found to be immediately corrected. >>Upon inspection it was found that the impacted soils had been excavated and placed in a roll off box. Cleanup was reported as being completed per the approved plan, no evidence was found to the contrary. |

Based on the inspection of the project area, it was determined that all requirements related to permanent stabilization and permit termination had been attained. I recommend termination of ESX14-125-0064. No violations were found during this inspection. |
