The 13 3/8” casing was set at 497 ft. on 6/22/17 and grouted to surface above a basket. Coal was found from 457 ft. to 462 ft. Baskets were set at 445 ft. and 450 ft. The 9 5/8” casing was set at 1971 ft. on 6/24/17 and cemented to surface 23 bbls returned. TD was at 6344 ft. on 6/28/17.

2792100 Primary Facility 2018-10-16 Incident- Response to Accident or Event No Violations Noted On 10/16/18 an incident inspection was conducted at the Malachosky Edward Well Site in response to an operator reported release. The following was observed:

>>On 10/15/18 Range Resources Representative Nate Smiell reported a 25 gallon spill resulting from a hole eroded in a drip line on the 7a GPU.

>>Site inspection found that excavation of visibly contaminated materials on the site had been completed. The operator indicated that roughly two and a half boxes of pad material were removed due to sumps having been dug as a precaution against forecasted rain events. Initial estimates of the amount spilled appear to be accurate. This constitutes a violation of 25 Pa. Code § 78.54 and 25 Pa. Code § 78.57(a) in that the operator failed to control regulated materials in a manner that prevents pollution to the ground and Waters of the Commonwealth.

>>The GPU was noted to not have appropriate secondary containment which constitutes a violation of 25 Pa. Code § 78.64(a) in that it did not have the required conta
