Moved onto this well on 6/29/17 and resumed drilling at 51 ft. KB. The 20” casing was set at 394 ft. on 6/30/17 and cemented to surface with 35 bbls returned. The 13 3/8” casing was set at 493 ft. on 7/1 17 and grouted to surface above a basket. Coal was found from 450 ft. to 463 ft. Baskets were set at 442 ft. and 450 ft. The 9 5/8” casing was set at 1988 ft. on 7/3/17 and cemented to surface with 15 bbls returned. KOP was at 6076 ft. Currently at 6366 ft. MD and drilling ahead.

2643315 Primary Facility 2017-09-13 Drilling/Alteration No Violations Noted Patterson-UTI Rig #288 moved onto this well on 7/18/17 and resumed drilling at 6917 ft. MD KB on 7/27/17. EOB was at 8612 ft. MD (7367 ft. TVD). TD was at 23,024 ft. MD (7207 ft. TVD) on 8/1/17. The 5 1/2" casing was set at 22,990 ft. Estimated TOC is at 5102 ft.
2753966 Primary Facility 2018-07-20 Routine/Complete Inspection No Violations Noted This well has been completed and is producing. The well and site appear to be well maintained. There is production equipment on site that is mislabeled.
2792107 Primary Facility 2018-10-16 Incident- Response to Accident or Event No Violations Noted On 10/16/18 an incident inspection was conducted at the Malachosky Edward Well Site in response to an operator reported release. The following was observed:

>>On 10/15/18 Range Resources Representative Nate Smiell reported a 25 gallon spill resulting from a hole eroded in a drip line on the 7a GPU.

>>Site inspection found that excavation of visibly contaminated materials on the site had been completed. The operator indicated that roughly two and a half boxes of pad material were removed due to sumps having been dug as a precaution against forecasted rain events. Initial estimates of the amount spilled appear to be accurate. This constitutes a violation of 25 Pa. Code § 78.54 and 25 Pa. Code § 78.57(a) in that the operator failed to control regulated materials in a manner that prevents pollution to the ground and Waters of the Commonwealth.

>>The GPU was noted to not have appropriate secondary containment which constitutes a violation of 25 Pa. Code § 78.64(a) in that it did not have the required conta

The site was found to be fully stabilized and in good condition with no issues found to prevent permit termination.
