2705062 Primary Facility 2018-03-07 Routine/Complete Inspection No Violations Noted Today I conducted a routine inspection at the Cogar well pad and the following was observed.

Proper signs were displayed identifying the site. EQT currently has a rig on site and is drilling 1 of 6 new wells. The well heads onsite are contained inside subsurface vaults covered with equipment for the drilling process and could not be visually inspected. The production tanks were in good condition no leaks , rust or defects observed. Secondary containment for the site was intact and maintained.

The site had some stabilization issues on the SW side of the access road. There was some slides forming inside and outside the LOD. I spoke with Brian Morris from EQT about the issue and we arranged a site inspection on 3/13/18 at 1:30pm to discuss the plan for stabilizing that area.
