>>The operator reported that a hole developed in a vac box/hose while transferring brine resulting in a spill of roughly 10 gallons outside of containment. Impacted pad materials were promptly removed and placed in disposal boxes on site. While this constitutes a violation of 5 Pa. Code 78.54 in that drilling fluids were spilled the violation is regarded as immediately corrected.

>>Multiple containment units around the site were noted to be damaged. Prompt repair of damage is strongly recommended. No violation.

>>A containment unit near the company trailer was noted to have significant oily residues inside and staining on the pad nearby indicated some possible spillage. No violation, remediation recommended.

2704692 Primary Facility 2018-03-08 Routine/Complete Inspection No Violations Noted On 3/8/18 an incident inspection was conducted of the Master Splinter well site in response to concerns regarding a report of a waste container testing being rejected from standard disposal for excessive radioactivity. The following was observed:

>> The site was in production with no significant activity at the time of inspection. The site was in good condition with no notable issues. No violation.

>>No waste containers were present on site at the time of inspection. No site personnel present were able to discuss the incident.
