I met with Hannah McAvoy on site to discuss the spill and associated remediation.
On 8/24/2018 at approximately 2245, a water truck experienced a hose clamp failure on its radiator which resulted in a release of approximately 9 gallons of antifreeze to the graveled well pad. Absorbents were placed on the affected area and Specialized Professional Services Inc. (SPSI) responded and remediated the impacted area placing it into one roll-off box. The affected area appeared to be remediated according to plan during the inspection. While inspecting the well site, the following was observed:
