2774187 Primary Facility 2018-09-12 Preoperation Inspection No Violations Noted On 9/12/18 a preoperation inspection of the Mizia James Well Site was conducted in response to an operator request. The following was observed:

>>Perimeter BMPs were in place and generally in good working order. Gaps between sections of CFS that may allow for sediment to leave the site were noted in two areas. No violation, remediation strongly encouraged.

Conclusion: >>The site appears to meet requirements to begin drilling and operations.

2777974 Primary Facility 2018-09-19 Drilling/Alteration No Violations Noted Lightning Energy auger rig drilled a 30" well bore and set 26" conductor casing at 40 ft. on 9/15/18.
2778625 Primary Facility 2018-09-24 Drilling/Alteration No Violations Noted Falcon Drilling Rig #27 moved onto this well on 9/22/18 and resumed drilling at 51 ft. KB. The 20" casing was set at 309 ft. on 9/24/18 and cemented to surface with 30 bbls returned. Currently at 339 ft. and waiting on cement.

Note: Approximately 1 gal. of drilling fluid was sprayed off containment as service truck was loading tank. Operator cleaning up with up.

2778866 Primary Facility 2018-09-21 Follow-up Inspection No Violations Noted On 9/21/18 a follow up inspection of the Mizia James Well Site was conducted in response to issues noted during the preoperation inspection related to site stabilization. The following was observed:

>>Site inspection found the all disturbed areas on site were temporarily stabilized with growth beginning.

>>Supplemental CFS had been added to multiple areas noted as potentially deficient during preoperation inspection.

2785283 Primary Facility 2018-10-05 Drilling/Alteration No Violations Noted Falcon Drilling Rig #27. The 13 3/8" casing was set at 478 ft. on 9/25/18 and cemented to surface with 11 bbls returned. The 9 5/8" casing was set at 1906 ft. on 9/27/18 and cemented to surface with 23 bbls returned. TD was at 6360 ft. on 9/28/18.
2826634 Primary Facility 2019-01-14 Routine/Complete Inspection No Violations Noted At the time of inspection the Operator was actively drilling the well. The Pioneer 78 rig was placed on secondary containment. No observation of accelerated erosion or sedimentation in relation to the well pad or access road. Act 9 site ID sign installed. ER plan, PPC plan, Secondary Containment plan and Waste Control and Disposal plan on site.
2838169 Primary Facility 2019-02-08 Drilling/Alteration No Violations Noted Pioneer Drilling Rig #78 moved onto this well on 1/3/19 and resumed drilling at 6375 ft. KB. KOP was at 6375 ft. EOB was at 8188 ft. MD (7188 ft. TVD). TD was at 24,337 ft. MD (6992 ft. TVD) on 1/14/19. The 5 1/2" casing was set at 24,303 ft. The estimated TOC is 5375 ft.
2847740 Primary Facility 2019-03-04 Routine/Complete Inspection In Compliance with Policy Inspection in response to a release reported by the Operator on 3/2/2019. The Operstor reported "On 3/2/2019 at 4:41 PM, Range Environmental Compliance was notified of a reuse water release to the graveled well pad. A contract water transfer company was in the process of transferring reuse water to the pump down tanks when a 3” hose sheared off at a cam-lock fitting. This resulted in approximately 10 barrels of reuse water being released to containment. The transfer pump was shut down and an onsite water truck immediately began to recover the released fluid. Further investigation revealed that approximately 30 gallons of reuse water was released to the graveled pad surface both through a hole in containment and by discharging over a containment berm. Some reuse water did discharge over the containment berm due to the velocity of the water coming out of the sheared hose. Specialized Professional Services Inc. (SPSI) responded to location and remediated the impacted area placing the material into two roll-off
