Operator indicated they would address the pit and correct the issue.

Site is secured with entrance gate and chain link fence around well head. Well is temporarily plugged. No activty on site at time of inspection. Inspected cellar, no visual evidence of gas leaking at well head. E&S controls appear to be in place and working . DEP off site at 11 am.
2095572 Primary Facility 2010-08-22 Plugging(Includes Plugged/Mined Through) No Violations Noted REceived call from Operator that after squeezing cement into 7" casing -observed bubbling at surface in 9x7" annulus at well head. DEP met with Company man. Observed steady bubbling in annulus. Used Altair 5 gas meter - detected 34% Methane. Spoke with Mgr. from Newfield Appalachia via conference call while on site. Discussed options to address bubbling. Plan is to expose top of 13 3/8" and monitor 9 5/8" x 7" and 9" x 13 3/8", and backside of 13 3/8" for the next two weeks while plugging operations proceed on Woodland Mgmt well and Crum Well. Upon completion of these well - Operator will return to Teeple and perform downhole work to stop gas movement.

On 9/18 performed perforations #5 and #6 AT DEPTHS 2298' and 2224', respectively. Could not establish injection rate. On 9/19 started milling 7" casing. At time of this inspection have @ 5' milled out. Plan to mill out 15 - 20' of 7" casing. Using 8.5" mill - hole is 8.75" in diameter. At this point no change in bubbling. After milling - may swab hole and perform shut in on 7" to see inf any pressure buildup. Discussed jet washing formations. No violations observed.
2106875 Primary Facility 2012-09-27 Plugging(Includes Plugged/Mined Through) No Violations Noted Steve Watson and Kevin Costello on site to meet with Mr. Pat Parsons (Co. Man for Newfield). Since last inspection have cut window in 7" casing at 2150' - 2160'. Jetted to formation for two hours with water. Still seeing gas at surface. Determined source of gas is above this zone. Set cement plug across window from 1740' - 2190'. On 9/23 picked up mill and attempted to mill 1260-1262. Had mechanical issues with mill and tripped out of hole. On 9/25 tripped back in hole to 1230' and attempted to cut window. Had problems again with mill and had to trip out of hole. At time of this inspection - tripping back in hole with new mill to 1231' and will begin cutting new window. Operator will provde update on plugging operations to DEP.
2110103 Primary Facility 2012-10-04 Plugging(Includes Plugged/Mined Through) No Violations Noted Follow up on plugging activities. Pat Parsons - Co. Man on site. On 9/28 - cut window 1233' - 1248. On 9/29 jetted window. On 9/30 - set cement plug 1075' - 1265'. On 10/1 - tagged plug and cut window @ 930'. Tried to mill and had mechanical issues and had to TOH. On 10/2 cut window @ 932'. On 10/3 cut window 890' - 899'. On 10/4 milled another 2' to 901' and currently circulating hole clean. After cutting last window - Operator closed in rams and pressured up 7"casing to 1500 lbs. There was no change in bubbling observed. Following squeeze job, operator will leave 1000lbs on cement for @ 2hrs. While on site had conference call with Newfield. Next step is to cut another window just below surface casing shoe at a depth of @ 770'. Operator will provide updates to DEP.
2110105 Primary Facility 2012-10-10 Plugging(Includes Plugged/Mined Through) No Violations Noted On 10/9 - Operator cut window at 500 - 507' in 7" and will be cutting a shallower window at @ 450'. According to Company Man, gas bubbling has slowed significantly after the cut was made at 500'. Plan going forward is to cement across windows. Opeartor will keep DEP updated on progress.
2110188 Primary Facility 2012-10-12 Plugging(Includes Plugged/Mined Through) No Violations Noted Have completed jetting of two windows (450-455' and 500-507'). Both were sand jetted using 300 lbs for @35-40 mins each - moving jet up and down window. Currently still seeing bubbling on 9 5/8" x 7" casing at a rate of @ 1 bubble per second. Will be cementing windows in 7" and will hold 1000 lbs on cement. During milling it was indicated that chunks of cement and metal were seen in returns to surface. Plan going forward is to cement across windows cut in 7". Will leave 1000 lbs on cement overnight. Then go in hole and tag TOC, swab hole clean, and fill remaining portion of 7" to surface with pea gravel. Will weld cap with permit number on well. The cellar will be filled in to base of the 9 5/8" casing top with fravel. Cellar will remain open and will continue to monitor condition of bubbling for next several weeks. DEP will re-visit the site routinely to observe and inspect well.
2110257 Primary Facility 2012-10-15 Plugging(Includes Plugged/Mined Through) No Violations Noted Since last inspection have cut two additional windows in 7" casing at 450' and 500'. Gas bubbling at the surface has decreased substantially. Over the weekend, a cement plug was set acorss cuts from 270' to 565'. 7" was topped off with gravel and capped. At this time cellar has been backfilled to just below 9 5/8" casing. top. At the time of inspection I ovserved bubbling in 9 5/8" x 7" annulus. bubbling was sporatic - approximately 1 every 5 - 10 seconds. A welded cap is on 7" with permit #. Cellar will remain open for the next couple of weeks. Secondary containment is still in place. Rig is stacked on site along with other remaining equipment. DEP will continue to re-visit site to monitor well.

Material recovered from the well pad has been placed in a wetland. This material needs to be removed immediately. Wetland damage needs to be repaired. Also, please provide documentation of the E & S inspections conducted over the last month.

Inspection conducted on 5/12/14. Inspection is to confirm Newfield’s assessment that the site meets the minimum requirements as defined in Chapter 102.22(C) for permanent stabilization which would thus fulfill the physical requirements necessary for the permits termination. The Department’s determination is that the site is in fact stabilized and meets the criteria necessary for the permit’s termination.
